of diagnostic tests, and it is appropriate to highlight its importance—particularly in the case of primary CNS lymphoma, which has decreased in prevalence since the advent of combination antiretroviral therapy [3–5]. Variability in nucleic acid amplification protocols between laboratories may result in varying levels of assay sensitivity. Our report aims, in part, to emphasize how EBV DNA PCR performs in a clinical setting that is commonly encountered in developed countries—specifically, in situations in which a commercial or commercially affiliated laboratory offers a novel diagnostic test for physicians in practice. These physicians often have no control over how the test is set up or performed. We believe our results have particular relevance for physicians practicing under such circumstances. We agree that, as for any nonstandardized test, the potential for variability should be carefully considered when EBV DNA PCR results are interpreted in the course of evaluation of patients with HIV infection and CNS disease.

Donor-derived West Nile virus infection in solid organ transplant recipients: report of four additional cases and review of clinical, diagnostic, and therapeutic features.

We describe four solid-organ transplant recipients with donor-derived West Nile virus (WNV) infection (encephalitis 3, asymptomatic 1) from a common donor residing in a region of increased WNV activity. All four transplant recipients had molecular evidence of WNV infection in their serum and/or cerebrospinal fluid (CSF) by reverse transcription polymerase chain reaction (RT-PCR) testing. Serum ...

sitive test. Of the patients with cases of West Nile virus identified in New York City in 1999 and 2000 and for whom a CSF sample was available, 95% had demonstrable IgM antibody (90% within 8 days of onset of symptoms) [4]. Residents in areas in which West Nile virus is endemic may have persistent IgM antibody from a previous infection that is unrelated to their current clinical illness, and, ...

West Nile virus-associated encephalitis in recipients of renal and pancreas transplants: case series and literature review.

Although West Nile fever is mild in the vast majority of infected persons, there is growing evidence that the disease may be more severe in the immunocompromised population. We describe 3 recipients of kidney or pancreas transplants who developed West Nile fever, 2 of whom had meningoencephalitis. As is the norm when treating serious infections in transplant recipients, a reduction of immunosup...

Prevalence of herpes simplex virus type 1 and 2 infections in renal transplant recipients and their renal function

Background and Objectives: Viral infections are a major problem for transplant recipients. Reactivation of viruses is associated with the use of strong immunosuppressive drugs. Herpes simplex virus infections in renal transplant patients may become severe without treatment. Although the prevalence and role of herpes simplex viruses 1 and 2 in bone marrow and lung recipients are well known; howe...

Material and Methods: the participants were 46 possible WNV case from Neka (13 women and 10 men) and Shiraz (10 women and 13 men).  IgG assay was ...
