Morphology of Precancer in Colitis
نویسنده
چکیده
There is general agreement that malignant change in ulcerative colitis mostly occurs in those patients whose large intestine is totally involved and have a history of symptoms for more than 10 years. Having thus defined the population of colitics most at risk the problem remains of identifying the individual patient destined to get carcinoma. There is yet no evidence that all the population at risk will eventually develop malignant change. It has been previously pointed out that some test is required which will decide that a patient with ulcerative colitis has entered a precancerous phase (Morson, 1966). The detection of epithelial changes suggestive of precancer or carcinoma in situ in rectal biopsies from patients with ulcerative colitis prompted an investigation into the incidence and extent of such changes in colectomy specimens removed for colitis with and without malignant change. The results of this enquiry are reported here as well as an analysis of the first nine patients in whom the diagnosis of precancer was made in a rectal biopsy and played an important part, together with clinical and radiological observations, in the subsequent decision to perform total proctocolectomy.

Rectal biopsy and precancer in ulcerative colitis.
Forty-seven patients with ulcerative colitis subjected to elective proctocolectomy were investigated with regard to the occurrence of precancerous changes in the rectal biopsy and the operation specimen. In the rectal biopsy from seven patients precancerous lesions were found and in the operation specimens from these patients precancer was also found in other parts of the colon. In five of thes...

متن کاملPrecancer and cancer in extensive ulcerative colitis: findings among 401 patients over 22 years.
Patients with extensive ulcerative colitis who do not need early surgery have been offered regular examination with the aim of detecting precancerous change (dysplasia) or early colorectal carcinoma. Outpatient visits with clinical examination, sigmoidoscopy, and biopsy were supplemented by two-yearly colonoscopy after the disease course reached 10 years. During the 22 year period from the begi...
متن کاملAbnormal lactic dehydrogenase isoenzyme patterns in ulcerative colitis with precancerous change.
The isoenzyme pattern of lactic dehydrogenase was measured in rectal biopsies from six patients with ulcerative colitis in whom precancerous histological changes had been independently recognized. There was a highly significant increase in the ratio of isoenzymes IV + V to I + II. This suggests that lactic dehydrogenase isoenzyme measurement may prove a valuable addition to histology in the rec...
متن کاملIn-situ and invasive carcinoma of the colon in patients with ulcerative colitis.
Four cases are described in which ulcerative colitis was complicated by carcinoma and in which the extent of the accompanying in-situ carcinoma (precancer) was determined. The extent of the in-situ carcinoma was very variable and in all cases the rectum was spared in whole or part. Search for in-situ carcinoma in rectal biopsies will fail to indicate the development of malignancy in some patien...
