Effects of amyl nitrite in aortic valvular and muscular subaortic stenosis.

چکیده
INHALATION of amyl nitrite causes abrupt vasodilatation and increase in cardiac output and intensifies the murmur of aortic stenosis.1 The transaortic systolic pressure gradient increases because of the increased ejection rate through the stenotic orifice. In muscular subaortic stenosis this effect is particularly marked,2-6 not only because of the increased ejection rate but also because the diminished heart size apparently causes increase in the severity of obstruction in the outflow tract. Glyceryl trinitrate has a similar effect.7 Amyl nitrite is a convenient stimulus for increasing the pressure gradient in muscular subaortic

Echocardiography in coexisting hypertrophic subaortic stenosis and fixed left ventricular outflow obstruction.
Echocardiographic study was done on four pediatric patients with coexisting hypertrophic subaortic stenosis (HSS) and fixed left ventricular outflow (LVO) obstruction. One had valvar aortic stenosis, two had discrete subaortic stenosis and one had coarctation of the aorta. Preoperatively, HSS was not suspected in any of these patients because of the classical findings of fixed LVO obstruction. ...
متن کاملHaemodynamic Effects of Changes in Blood Volume in Hypertrophic Obstructive Cardiomyopathy.
Several physiological and pharmacological influences are known to alter the degree of obstruction in hypertrophic obstructive cardiomyopathy (hypertrophic subaortic stenosis, muscular subaortic stenosis). Increased left ventricular-arterial systolic gradients in the beats following a ventricular ectopic contraction and following the administration of isoprenaline and of digitalis are probably r...
متن کاملAcute haemodynamic effects of practolol in patients with idiopathic hypertrophic subaortic stenosis.
Ten patients with idiopathic hypertrophic subaortic stenosis were studied with right and transseptal left heart catheterization. All patients had the expected increases in subvalvular systolic gradients with isoprenaline infusion, amyl nitrite inhalation, exercise, and performance of the Valsalva manoeuvre. These stimulatory procedures were repeated after intravenous infusion of practolol (6oo ...

were performed to induce or increase the functional outflow tract obstruction; 150 pug/kg of propranolol was administered intravenously, and the stimulating maneuvers were repeated. Outflow obstruction was latent (present only with stimulation) in four patients, labile (present intermittently at rest) in five patients, and persistent and severe in two patients. Propranolol abolished or signific...
متن کاملThe arterial pressure pulse in muscular subaortic stenosis.
Brock (1957, 1959) using the term "Functional obstruction of the left ventricle" was the first to describe muscular subaortic stenosis. Since then, a number of reports (Bercu et al., 1958; Morrow and Braunwald, 1959; Brent et a!., 1960; Goodwin et al., 1960; Braunwald et al., 1960; Brachfeld and Gorlin, 1959, 1961; Pare et al., 1961; Boiteau and Allenstein, 1961) have appeared concerning this c...
