Mechanics of left ventricular contraction in chronic severe mitral regurgitation.

چکیده
The mechanics of left ventricular contraction were studied during diagnostic cardiac catheterization using high-speed cineangiography in 11 patients with severe chronic mitral regurgitation. Compared with a group of previously studied normal subjects, most of the patients with mitral regurgitation demonstrated a reduced velocity of shortening (Vf) during ejection at maximum wall stress, average = 1.01 circumferences/sec (circ/sec) (range 0.64-1.47 cire/sec). Maximum and mean V0f values also were reduced in these patients, averaging 1.40 and 0.94 circ/sec, respectively. These findings are in contrast to those in acute experimental mitral regurgitation, and to observations of normal shortening velocities in chronic experimental volume overloading, in which left ventricular contraction velocity is augmented. Compared with normal subjects, patients with mitral regurgitation had significantly larger left ventricular end-diastolic circumferences and volumes, and higher total left ventricular stroke volumes. The mean regurgitant volume was 41% of the total stroke volume, of which an average of 46% was ejected into the left atrium prior to aortic valve opening. The ejection fraction and extent of fiber shortening were normal in all but two patients despite depressed shortening velocities in most. It is concluded that analysis of velocity in the ejecting phase of left ventricular contraction is useful in detecting apparent alterations in inotropic state in the face of the altered loading conditions accompanying chronic mitral regurgitation. Further, favorable unloading conditions early during systole in patients with mitral regurgitation appear to mask the effects of a depressed inotropic state on the pumping function of the heart.

Echocardiographic assessment of left ventricular function in mitral valve disease.
Echocardiography was used to study left ventricular size and contraction in 90 patients with isolated mitral valve disease--47 patients with mitral stenosis, 26 with mixed mitral valve disease and 89 with mitral regurgitation. Left ventricular measurements included the end-diastolic internal dimension (LVIDd), mural thickness (PWTd), an index of circumferential myocardial contraction--fractiona...

Introdution: Ischemic mitral regurgitation (IMR) remains one of the most complex and unresolved aspects of ischemic heart disease that the impact of percutaneous coronary intervention (PCI) on improvement of intensity of ischemic mitral regurgitation is not well clarified. Patients with coronary artery diseases and ischemic mitral regurgitation have a worse prognosis than the patients with coro...
متن کاملHigh-risk Coronary Artery Bypass Grafting and Mitral Valve Replacement in a HIV Positive Patient
Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...
متن کاملNormal myocardial function in severe right ventricular volume overload hypertrophy.
Severe left ventricular volume overloading causes myocardial and cellular contractile dysfunction. Whether this is also true for severe right ventricular volume overloading was unknown. We therefore created severe tricuspid regurgitation percutaneously in seven dogs and then observed them for 3.5-4.0 yr. All five surviving operated dogs had severe tricuspid regurgitation and right heart failure...
متن کاملChronic Canine Mitral Regurgitation
The mechanism by which ft blockade improves left ventricular dysfunction in various cardiomyopathies has been ascribed to improved contractile function of the myocardium or to improved ,8-adrenergic responsiveness. In this study we tested two hypotheses: (a) that chronic 13 blockade would improve the left ventricular dysfunction which develops in mitral regurgitation, and (b) that an important ...
