Late-Onset, Advanced Scapular Notching Associated with a Lateral Offset Reverse Arthroplasty Design - A Case Report

چکیده
Reverse shoulder arthroplasty, introduced by Grammont et al. [1] in 1987, is commonly associated with scapular notching, [2] an impingement of the humeral cup against the scapula’s inferior border. Although the clinical significance of notching has been debated, 2 studies reported less successful clinical outcomes in its presence [2,3]. A laboratory study showed that a varus humeral neck-shaft angle and a lateral glenosphere offset reduce inferior scapular impingement [4]. The DJO Encore reverse shoulder prosthesis (Encore Medical, Austin, Texas) has a varus, 135° humeral neck-shaft angle and a lateralized offset glenosphere component [5]. A literature search found 1 study [6] (5-year follow-up) and 1 case report [7] describing notching associated with this reverse arthroplasty design, but neither reported more than a Sirveaux [2] Grade 1 notching.

Prosthetic design of reverse shoulder arthroplasty contributes to scapular notching and instability
AIM To evaluate whether implant design, glenoid positioning, and other factors influenced instability and scapular notching in reverse total shoulder arthroplasty. METHODS We retrospectively reviewed records of patients who had undergone reverse total shoulder arthroplasty by the senior author from July 2004 through October 2011 and who had at least 24 mo of follow-up. The 58 patients who met...
متن کاملInferior glenosphere placement reduces scapular notching in reverse total shoulder arthroplasty.
Scapular notching is a common complication after reverse shoulder arthroplasty and has been associated with poor clinical outcomes. Factors associated with notching include neck shaft angle and glenosphere position. The goal of this study was to evaluate the incidence of notching with an eccentric glenosphere that allows for inferior offset as well as its effect on clinical outcome. The charts ...
متن کاملHumeral-sided Radiographic Changes Following Reverse Total Shoulder Arthroplasty
Background: We sought to characterize humeral-sided radiographic changes at a minimum of 2 years after reverseshoulder arthroplasty (RSA) to determine their association with specific implantation techniques. Methods: The immediate and most recent postoperative anteroposterior radiographs of 120 shoulders with primary RSAand a minimum of 2-years of radiographic follow-up were an...
متن کاملEffect of lateral offset center of rotation in reverse total shoulder arthroplasty: a biomechanical study.
BACKGROUND Lateral offset center of rotation (COR) reduces the incidence of scapular notching and potentially increases external rotation range of motion (ROM) after reverse total shoulder arthroplasty (rTSA). The purpose of this study was to determine the biomechanical effects of changing COR on abduction and external rotation ROM, deltoid abduction force, and joint stability. MATERIALS AND ...
متن کاملInfraglenoidal scapular notching in reverse total shoulder replacement: a prospective series of 60 cases and systematic review of the literature
BACKGROUND The impact of infraglenoidal scapular notching in reversed total shoulder arthroplasty (RTSA) is still controversially discussed. Our goal was to evaluate its potential influence on subjective shoulder stability and clinical outcome. We hypothesized that subjective instability and clinical outcome after implantation of RTSA correlates with objective scapular notching. METHODS Sixty...
