Endomyocardial biopsy: the balance of success and failure.
نویسنده
چکیده
ENDOMYOCARDIAL BIOPSY (EMB) has not fulfilled its promise. But, it may soon. It was a reasonable early expectation that the technique would provide an understanding of myocardial disease that would lead to effective treatment in most patients. This expectation has not been met. Instead, EMB has served an unforeseen medly of purposes, and has often found itself involved in odds and ends of cardiology. Yet, in circumscribed areas EMB has been successful in that it has benefited patients and clarified disease processes. In fact, the balance between successes and failures of EMB is close, and the ultimate place of EMB in cardiologic practice will be determined by the change in this balance in the near future.

Long Sheaths for Endomyocardial Biopsy via Femoral Approach
Background-Endomyocardial biopsy has long been a useful tool to evaluate myocardial diseases. It is a gold standard in diagnosing cardiac rejection and certain types of myocarditis, but there is always a risk for related complications. We have assessed the safety and feasibility of applying long sheaths in endomyocardial biopsy via the femoral approach. Methods-We performed endomyocardial biops...

A histopathological index of contractility failure, which was reported to be accurate for the diagnosis of idiopathic dilated or congestive cardiomyopathy in Japan, was used to assess endomyocardial biopsy specimens from 41 Dutch patients with suspected dilated cardiomyopathy. The calculated index had a sensitivity of 62.5% and a specificity of 68% for idiopathic dilated or congestive cardiomyo...
متن کاملManaging the patients of heart failure with the data from cardiac MRI and endomyocardial biopsy
Background As for chronic heart failure, there are wide varieties of spectrum of underling etiological processes. For the management of heart failure, characterization of tissue damage and clarifying the cause of disease is useful. Cardiac magnetic resonance (CMR) imaging is useful tool for evaluation of histopathology and cardiac function in myocardial disease. We compared the imaging of CMR w...
متن کاملCritical assessment of diagnostic value of endomyocardial biopsy. Assessment of cardiac biopsy.
Right or left ventricular endomyocardial biopsy with the Konno or the Olympus bioptome was attempted in 73 patients aged 5 months to 61 years, with 82 per cent success. Light and electron microscopy showed non-specific features in over half the biopsies with new or diagnostically useful information in only 10 per cent of cases, usually as a result of electron microscopy. The method is safe but ...
متن کاملUpdate on Myocarditis and Inflammatory Cardiomyopathy: Reemergence of Endomyocardial Biopsy.
Myocarditis is defined as an inflammatory disease of the heart muscle and is an important cause of acute heart failure, sudden death, and dilated cardiomyopathy. Viruses account for most cases of myocarditis or inflammatory cardiomyopathy, which could induce an immune response causing inflammation even when the pathogen has been cleared. Other etiologic agents responsible for myocarditis includ...
