چکیده
results sixty two patients had dka. the comparison of apache ii score among two groups of the patients did not show any significant difference (p = 0.597). there was no suitable cut-off point for apache ii score to predict dka. conclusions apache ii score cannot be applied in the predicting of dka in hyperglycemic patients admitted in ed. objectives the present study aims to evaluate the efficacy of acute physiology and chronic health evaluation ii (apache ii) scoring in predicting the critically illness in the hyperglycemic patients referring to the ed. patients and methods we performed a prospective cohort study in an ed. one hundred eighty one patients older than 18 years with hyperglycemia were included in our study. following the primary evaluation, the subjects were divided into dka and non-dka patients. apache ii scores were calculated for all patients and then compared to each other. we determined predictive value, sensitivity, specificity and cut-off points of apache ii score for dka. background diabetic ketoacidosis (dka) is an acute and life-threatening complication in diabetic patients. the current diagnostic criteria of dka are metabolic acidosis, blood glucose level greater than 250 mg/dl and the presence of ketones in serum or urine. dka patients referring to the emergency department (ed) are usually ill.

متن کاملManagement of Diabetic Ketoacidosis in the Emergency Department
Diabetic ketoacidosis results from deficient insulin action and increased action of hormones such as catecholamines, glucagon, glucocorticoids, and growth hormone, which are produced during stress and which antagonize insulin's actions. Diabetic ketoacidosis is associated with a relatively high mortality rate. Treatment consists of appropriate fluid resuscitation, insulin infusion, adjustments ...
متن کاملPredictors of Recurrent Hospital Admission for Patients Presenting With Diabetic Ketoacidosis and Hyperglycemic Hyperosmolar State
BACKGROUND Diabetic ketoacidosis (DKA) and hyperglycemic hyperosmolar state (HHS) are two serious, preventable complications of diabetes mellitus. Analysis of variables associated with recurrent DKA and HHS admission has the potential to improve patient outcomes by identifying possible areas for intervention. The aim of this study was to evaluate potential predictors of recurrent DKA or HHS adm...

Diabetic ketoacidosis (DKA) and hyperglycemic hyperosmolar nonketotic syndrome (HHNS) are life-threatening acute metabolic complications of diabetes mellitus. Although there are some important differences, the pathophysiology, the presenting clinical challenge, and the treatment of these metabolic derangements are similar. Each of these complications can be seen in type 1 or type 2 diabetes, al...

Diabetic ketoacidosis (DKA) and the hyperglycemic hyperosmolar state (HHS) are potentially fatal hyperglycemic crises that occur as acute complications of uncontrolled diabetes mellitus. The authors provide a review of the current epidemiology, precipitating factors, pathogenesis, clinical presentation, evaluation, and treatment of DKA and HHS. The discovery of insulin in 1921 changed the life ...

Antecedent symptoms of polyuria and polydipsia, lassitude, blurred vision, and mental status changes may predominate the clinical picture. With DKA, abdominal pain and tachypne...
