چکیده

one of the routine methods for diagnosis and treatment of cholelithiasis is endoscopic retrograde cholangiopancreatography. it is an effective method but is associated with some complications (9.8%) which the most common is pancreatitis (5.4%). one of the rare complications is basket impaction in the common bile duct which is treated with different methods. a 58 years male history of laparoscopic cholecystectomy was referred for endoscopic retrograde cholangiopancreatography. during the extraction of stones, the basket was impacted and the distal part was dissociated from the proximal part. stent was placed in the common bile duct and with laparatomy and choleducotomy, bascket with stent and remained stone were extracted and choleducoduodenostomy was done. there are several suggested methods for basket removal; as stenting to relieve the obstruction and possibly allow for spontaneous passage of the impacted stone and basket, endoscopic lithotripsy by percutaneous transhepatic approach, extracorporeal shockwaves to fragment an ampullary impaction, laparoscopic exploration, stone dissolution and surgical intervention. treatment must be tailored to the patient's clinical situation and condition, and the modalities that are accessible. â  â

A forward-pulling technique for complicated Trapezoid basket impaction during retrieval of common bile duct stones.

Although basket impaction rarely occurs during retrieval of common bile duct stones, it is a potentially serious event [1]. The Trapezoid RX wire-guided retrieval basket (Boston Scientific Corp., Marlborough, Massachusetts, USA) has an emergency release feature, whereby the tip is designed to disengage, thereby facilitating basket removal and reducing the risk of impaction [2]. However, we enco...

Argon plasma coagulation for the resolution of basket impaction from large common bile duct stones

One of the most common indications for endoscopic retrograde cholangiopancreatography (ERCP) is choledocholithiasis – the occurrence of stones in the common bile duct (CBD). However, in approximately 5% to 10% of cases, the procedure fails, especially in a patient with large or impacted stones [1]. An uncommon adverse event during ERCP with CBD stone clearance is impaction of the basket-stone c...

Laser lithotripsy to treat basket impaction during mechanical lithotripsy of a pancreatic duct stone.

CONTEXT Though uncommon, pancreatic duct stones can cause significant discomfort and morbidity. Endoscopic removal of pancreatic stones may decrease intraductal pressure by augmenting ductal drainage. Endoscopy is shown to be most effective when used early in the course of the disease. Endoscopic methods such as mechanical lithotripsy are often successful in removing the majority of pancreatic ...

Long-term follow-up of common bile duct diameter after endoscopic sphincterotomy in patients with common bile duct stones.

BACKGROUND AND AIMS To determine the time to normalization of common bile duct (CBD) diameter after endoscopic sphincterotomy and stone extraction in patients with choledocholithiasis. METHODS Patients with CBD dilation due to choledocholithiasis were enrolled. CBD diameter was measured by transabdominal ultrasonography before, and repeated after one, three, six and twelve months after endosc...
