چکیده

intestinal normal flora can become reservoirs of antibiotic resistance genes present among the strains responsible for nosocomial infections. it is suggested that gram negative intestinal bacterial flora have increased capacities to obtain antibiotic resistance genes and therefore can act as main reservoirs for transfer of resistance genes to other pathogenic bacteria. this study aimed to compare fecal carriage of clinically important resistance markers for more frequent members of enterobacteriacae between nondiarrheal and community associated diarrheal patients (control group) versus their counterparts from the patients with nosocomial infections (case group). 261 stool and 190 clinical samples were collected from outpatient and hospitalized patients from 6 hospitals in tehran, iran. the samples were cultured on macconkey agar plates and colonies were identified by standard biochemical methods. antibiotic sensitivity testing of the isolates against 13 antibiotics was performed according to the clsi guideline using the disk diffusion method.   among stool and clinical samples, more frequent identified enterobacteriaceae bacteria were included e. coli (58.99/ 3.15%), klebsiella spp . (22.61/7.36%), and other members of enterobacteriaceae (8.86/1.06%), respectively. overall, resistance against four of the main antibiotics (3 th and 4 th generation cephalosporins, gentamicin, imipenem, and ciprofloxacin) was significantly higher among the case group (50-75% versus 10-14%). analysis of these results showed similar dissemination of resistance phenotypes among the isolates from the control group in ranges of 1.5-7.6% and 4.4% for e. coli and klebsiella spp., respectively. our results suggested that the fecal carriage of resistant phenotypes related to the β-lactam antibiotics in e. coli and klebsiella spp. in compare to the clinical isolates is rapidly increasing. this may be caused by dissemination of β-lactamase producing e. coli in the community from the hospitals. there were no significant correlations between the two groups of the samples, as the clinical samples had shown 3 to 7 folds excess resistance phenotypes. surveillance studies of the resistance patterns among the samples from different regions will provide awareness about dissemination of these bacteria within the community as reservoirs of main resistance markers.

Risk Factors for Fecal Carriage of Extended-Spectrum Beta-Lactamase Producing Escherichia coli and Klebsiella spp. in the Community

Materials and Methods: A total of 928 stool samples admitted to the laboratory during a four-month period were included in the study. Samples were diluted in saline and cultured in two EMB agar plates supplemented with either 1 μg/ml cefotaxime or 1 μg/ml ceftazidime. All isolates that grew were identified to the species level. E. coli and Klebsiella spp. strains were tested for ESBL production...

Fecal carriage of extended-spectrum beta-lactamase-producing Escherichia coli and Klebsiella spp. in a Turkish community.

BACKGROUND/AIM The aim of this study was to determine the prevalence of fecal carriage of extended-spectrum beta-lactamase (ESBL)-producing bacteria, enzyme types, and risk factors affecting colonization. MATERIALS AND METHODS A total of 576 stool samples from outpatients were examined between October 2012 and May 2013. Screening was done with selective EMB plates. ESBL were detected by doubl...

Evaluation of Efflux pump activity among Uropathogenic Escherichia coli and Klebsiella pneumonia multiple- Drug Resistance isolates

Antibiotic resistance is a phenomenon in which antibiotic used to treat bacteria becomes useless due to resistance mechanism. Increased drug resistance and occurrence of Multiple Drug Resistance in bacteria specificity nosocomial and Urinary Tract Infection bacteria has reduced the possibilities of treating these infectious diseases. Efflux pumps are one of the major mechanism of MDR in bacteri...

Fecal carriage of Escherichia coli harboring extended-spectrum beta-lactamase (ESBL) genes by sheep and broilers in Urmia region, Iran

Background: There is a growing concern on the impact of the presence of extended-spectrum β-lactamase (ESBL) producing Escherichia coli isolated from animals on public health. OBJECTIVES: The aim of this study was to investigate the presence of three classes of ESBL genes in E. coli isolates from sheep and broilers at a slaughter in Urmia region, Iran. METHODS: A total of 111 E. coli isolates w...

Correlation of quinolone-resistance, qnr genes and integron carriage in multidrug-resistant community isolates of Klebsiella spp.

Objective(s): Plasmid-mediated quinolone resistance (PMQR) determinants and integrons have a considerable contribution to bacterial drug resistance in Gram-negative pathogens. We studied the prevalence of PMQR genes and integron carriage in multidrug-resistant community isolates of Klebsiella spp.Materials and Methods: Two hundred and fi...

Antimicrobial drug resistance of fecal Escherichia coli and Salmonella spp. isolates from United States dairy cows.

Antimicrobial resistance is a growing concern for public and animal health. Threats to public health could come from the transfer of pathogens from animals to people via indirect contact such as through food or by direct contact with animals. In addition, concern has been raised for the potential transfer of resistance determinants from animals to humans through commensal bacterial flora such a...
