گزارش یک مورد حامل بدنی تریاک (body packer) و درمان موفقیتآمیز وی

چکیده
مرد 55 ساله مجهولالهویهای با کاهش سطح هوشیاری، کاهش تعداد تنفس، کبودی پوست و مردمکهای تنگ از یک پایانه مسافربری به بیمارستان منتقل شد. بیمار در معاینات عصبی، علائم یک طرفه نداشت و انتهاها کبود بودند. بیمار اندکی بعد از دریافت گلوکز هیپرتونیک و نالوکسان دچار تشنج، افت فشار خون و آپنه گردید و در نتیجه تحت لولهگذاری داخل نای و تهویه مکانیکی قرار گرفت. آزمایشات اولیه، اسیدوز تنفسی و هیپوکسمی را نشان داد. بعد از انجام سایر آزمایشات و سیتی اسکن، بیمار مجدداً تحت درمان با نالوکسان قرار گرفت و هوشیاری نسبی پیدا کرد. با شک به بلع بسته، برای بیمار عکس ساده شکم انجام شد و وجود بستههای متعدد تأیید گردید. درمان با پلیاتیلن گلیکول به میزان 2 لیتر در ساعت همراه با انفوزیون نالوکسان و شارکول سریال برای بیمار آغاز شد و بیمار بستهها را دفع نمود و در نهایت با حال عمومی خوب مرخص گردید.
