Visceral Mycotic Aneurysm: Superior Mesenteric Artery Aneurysm Caused by <b><i>K. pneumoniae</i></b>

We herein report a rare case of mycotic aneurysm the superior mesenteric artery caused by <i>Klebsiella pneumoniae</i>. A 66-year-old man, known hypertension and aorto-oesophageal fistula with stented aorta in 2010 2018, presented to emergency department multiple times over 2 months severe postprandial abdominal pain associated vomiting fever. On his last presentation, obtained blood cultures grew ESBL positive <i>K. pneumoniae</i> repeated computed tomography (CT) showed growing at origin ileocecal branch measuring 17 × 10 mm (the was 8 7.5 CT angiography on previous admission). Extensive workup did not reveal underlying cause aneurysm, thus we believe be infected aortic stent, leading bacteraemia vegetations causing aneurysm. The management plan placed multidisciplinary team consisting vascular surgeons infectious disease specialists along review from dietician evaluate patient’s nutritional status. patient started total parenteral nutrition due antibiotic therapy according team’s recommendation. He underwent surgical resection which thrombosed jejunoileal area. histopathology resected tissue demonstrated inflammatory artery. Following surgery, continued discharged 13th post-operative day follow-up appointments surgery clinic.
