V-021 MIDLINE RESTORATION VIA EMILOS TECHNIQUE

Abstract Aim In recent years, numerous minimally invasive techniques have been developed for the repair of midline hernias associated with diastasis recti. The eMILOS technique (Endoscopic mini/less open sublay technique) allows mesh placement in retromuscular space through a small trans-hernial incision without entering abdominal cavity. Material & Methods We present case 45 year-old male who complained umbilical bulge and pain. On physical exam, an hernia measuring 2cm was observed, 5cm Midline restoration via approach decided. this technique, transhernial endoscopic is performed two 5mm ports on both flanks, allowing complete dissection space. First, performed. hernial content reduced posterior sheath closed. Then, we place homemade device (consisting wound protector, surgical glove port) to perform approach. When completely dissected, anterior sheaths are closed barbed sutures. A large heavy polypropylene placed fixed absorvable tackers glue. Afterwards, step over taked out, fascia running suture umbilicus scar fixed. Results patient presented favorable evolution, no recurrence year postoperatively. Conclusions safe effective alternative treatment recti defects.
