V-010 ROBOTIC ASSISTED BILATERAL INGUINOSCROTAL HERNIA REPAIR: STEP BY STEP

Abstract Introduction Robotic surgery in the field of abdominal wall is exponentially growing. In this video we will present a step by robotic approach for bilateral inguinoscrotal hernia repair. Clinical case A 79 year-old male with history hypercholesterolemia was diagnosed inguinal hernia. On physical examination, right (EHS classification L3P) and left M1P) were diagnosed. Minimally invasive assisted DaVinci Xi robot decided. Under general anesthesia, supraumbilical 8-mm trocar located 2 other trocars are positioned 8 cm laterally. Monopolar scissors bipolar grasps introduced. Laparoscopic examination confirmed diagnosis. First, peritoneum dissected, allowing to access preperitoneal space, dissecting medial lateral compartiment, reaching pectineal ligament. Identification reduction sac performed, individualizing preserving structures. Critical view miopectineal orifice ensured. Contralateral procedure performed. 15×15 Poplipropilene mesh at fixated 3/0 PDS stitches Fixation done superior section mesh. Peritoneal flaps closed V-lock suture. Conclusion safe feasible repair patients specially selected (BMI <30, hernias). Further studies needed order clarify its role minimally surgery.
