چکیده
Osi?gni?cie remisji wolnej od leczenia (TFR) sta?o si? w kilku ostatnich latach nowym, wa?nym celem terapii chorych na przewlek?? bia?aczk? szpikow? (CML). Doprowadzenie do d?ugotrwa?ej i g??bokiej molekularnej, a nast?pnie próba odstawienia inhibitora kinaz tyrozynowych (TKI) jest strategi? szczególnie atrakcyjn? dla m?odszych oraz tych, u których stosowane TKI wywo?uj? przewlek?e dzia?ania niepo??dane znacz?co obni?aj?ce jako?? ?ycia. Wyniki wielu bada? klinicznych wskazuj? mo?liwo?? uzyskania trwa?ej TFR oko?o 50% chorych. U pozosta?ych dochodzi utraty wi?kszej odpowiedzi molekularnej (rzadziej ca?kowitej cytogenetycznej [CCyR]), co stanowi wskazanie ponownego wdro?enia leczenia. Procedur? uznaje za ca?kowicie bezpieczn?. W toku prowadzonych nie odnotowano progresji ani zgonu spowodowanego odstawieniem TKI. Poni?ej zaprezentowano przypadek chorego, którego dosz?o CCyR po odstawieniu nilotynibu. Ponowne wdro?enie tego leku szybko doprowadzi?o odzyskania DMR (MR4).
