Use of psychotropic medications during pregnancy and the postpartum period: Review on Recent Works and Clinical Scenarios

Introduction The effects of antidepressant and antipsychotic medications in the perinatal period both mothers children have been a subject interest for many decades. Risks benefits should be considered according to illness stage, trimester pregnancy/ postpartum period, neonatal outcomes. Objectives Our goal was summarize knowledge about use antidepressants antipsychotics period. To illustrate complexity treatment decisions with clinical reports. Methods Review: A narrative review carried out using PubMed database including papers published 2022. Evidence risks is presented. Search terms: OR AND (perinatal pregnancy postpartum). Case reports (5 scenarios): (1) pre-pregnancy counselling, (2-4) first-, second- third-trimester pregnancy, (5)postpartum/breastfeeding. Results (1)Depression/antidepressants. Treating maternal depressive symptoms associated lower risk complications. Although placental passage sertraline low, drug monitoring recommended. Antidepressant preterm delivery low weight at birth. (2)Psychosis/Antipsychotics. Antipsychotic intrauterine exposure not significantly increased major congenital malformations. Minimum effective doses are (1)Pre-pregnancy counselling. Schizoaffective disorder receiving perphenazine, quetiapine lithium. (2)First-trimester pregnancy. Discontinuation disorder. (3-4)Second/third trimester. Occurrence anxiety posttraumatic stress (5)Postpartum/Breastfeeding. antidepressants.
