The most stable pinning configurations in transverse supracondylar humerus fracture fixation in children: A novel three-dimensional finite element analysis of a pediatric bone model

Purpose This study aimed at finding out the effect of exit height, trajectory and number pins on stability cross divergent-lateral used in fixation extension-type, transverse supracondylar humerus fracture (SHF) children, based finite element analysis. Methods Distal model consisting ossific nucleus capitellum (ONC) distal cartilage a 6-year-old boy was developed via three-dimensional modeling. Various pinning models with either two or three were simulated an SHF tested six loading directions. Results Two-cross 2-divergent-lateral more stable against torsional translation forces respectively, while 3-cross most all forces. The exiting upper border metaphyseal-diaphyseal junction (MDJ) had best stiffness among 2-cross pins, lateral middle third ONC pin provided 2-lateral pins. A olecranon fossa greatly enhanced Conclusion For typical fractures, are found to be superior 2-divergent only Pins MDJ provides mechanical Two-divergent-lateral going through for these fractures. Three-cross however offer both offers important clues preoperative evaluation management extension-type fractures children.
