Introduction: Hypokalemia is common disorder characterized by low plasma potassium levels (<3.5 mEq / L). can be caused genetic disorders. Bartter syndrome and Gitelman are rare disorders that cause damage to the tubular kidneys. The of hypokalemia must determined analyzing diagnosis algorithm hypokalemia.Case Illustration: A 27-year-old woman was brought emergency room with complaints weakness in both legs since 1 day ago. Obtained a history chronic 5 years No thyroid disease, never taking diuretic drugs. patient calm. Vital signs: BP: 110/60, regular pulse 88x/minute, temperature: 36.7°C, respiratory rate 14x/minute, oxygen saturation 99% air. ECG showed Normal sinus rhythm normal T wave. Laboratory findings severe 1.7 mEq/L, increased urine (71.1 mmol/24 hours), sodium 306 hours, chloride (342 magnesium were (1.91 mg/dL). KCl infusion given correct electrolyte imbalance condition.Discussion: : Several examinations performed confirm condition. this suspected lead syndrome, because there an increase urinary excretion, normotensive conditions, no suspicion metabolic acidosis, symptoms nausea vomiting drugs usage. Keywords: Hypokalemia,
