The clinical relevance and mechanism of skeletal muscle wasting

چکیده
Skeletal muscle wasting occurs in both chronic and acute diseases. Increasing evidence has shown this debilitating process is associated with short- long-term outcomes critical, cancer surgical patients. Both quantity quality, as reflected by the area density of a given range attenuation CT scan, impact patient prognosis. In addition, ultrasound bioelectrical impedance analysis (BIA) are also widely used assessment body composition due to their bedside viability no radioactivity. Mechanism researches have revealed complicated pathways involved wasting, which include altered IGF1-Akt-FoxO signaling, elevated levels myostatin activin A, activation NF-?B pathway glucocorticoid effects. Particularly, central nervous system (CNS) been proven participate regulating various conditions, such infection tumor. Several promising therapeutic agents under developing treatment atrophy, antagonist, ghrelin analog, non-steroidal selective androgen receptor modulators (SARMs). Notably, nutritional therapy still fundamental support combating wasting. However, optimizing tailored nutrition regimen relies on accurate metabolism measurement large clinical trials future. Here, we will discuss current understanding potential practice.

Acute skeletal muscle wasting in critical illness.
IMPORTANCE Survivors of critical illness demonstrate skeletal muscle wasting with associated functional impairment. OBJECTIVE To perform a comprehensive prospective characterization of skeletal muscle wasting, defining the pathogenic roles of altered protein synthesis and breakdown. DESIGN, SETTING, AND PARTICIPANTS Sixty-three critically ill patients (59% male; mean age: 54.7 years [95% CI...
متن کاملThe ubiquitin-proteasome system and skeletal muscle wasting.
The ubiquitin-proteasome system (UPS) is believed to degrade the major contractile skeletal muscle proteins and plays a major role in muscle wasting. Different and multiple events in the ubiquitination, deubiquitination and proteolytic machineries are responsible for the activation of the system and subsequent muscle wasting. However, other proteolytic enzymes act upstream (possibly m-calpain, ...
متن کاملRunx1 prevents wasting, myofibrillar disorganization, and autophagy of skeletal muscle.
Disruptions in the use of skeletal muscle lead to muscle atrophy. After short periods of disuse, muscle atrophy is reversible, and even after prolonged periods of inactivity, myofiber degeneration is uncommon. The pathways that regulate atrophy, initiated either by peripheral nerve damage, immobilization, aging, catabolic steroids, or cancer cachexia, however, are poorly understood. Previously,...
متن کاملPrevention of Skeletal Muscle Wasting: Disuse Atrophy and Sarcopenia
Skeletal muscle plays a considerable role in health and disease. Muscle mass is essential for health and survival and plays a major role in mobility as well as morbidity and mortality. There is continual synthesis and degradation of proteins as part of normal metabolism and homeostasis. Equally remarkable, is the characteristic of plasticity allowing muscle to change and adapt depending on the ...
متن کاملCancer cachexia-anorexia syndrome and skeletal muscle wasting
Background. Cachexia-anorexia syndrome is a common and important indicator of cancer. It occurs in 30% to 80% of cancer patients. Cachexia means “bad condition” and may be present in the early stages of tumor growth, before any signs of malignancy. Cancer cachexia is a syndrome of progressive body wasting, characterized by loss of adipose tissue and skeletal muscle mass. In most cancer patients...
