Stroke Severity Mediates the Association Between Socioeconomic Disadvantage and Poor Outcomes Among Patients With Acute Ischemic Stroke

چکیده
Background Impact of mediating factors on the relationship between socioeconomic disadvantage and outcomes among patients with acute ischemic stroke has not been well characterized. Methods Data were extracted from electronic medical records, 90‐day modified Rankin scale (mRS) scores collected as part a prospective registry. Exact patient addresses geocoded characterized using Area Deprivation Index (ADI) ranks. The ≥3 categorized poor outcomes. Logistic regression models (adjusted for treatment intravenous tissue plasminogen activator or intraarterial therapy, sociodemographics, comorbidities) fitted to compute adjusted odds ratios (aORs) 95% CIs total effect high ADI In‐hospital mortality (versus survived) unfavorable favorable) discharge disposition also evaluated Structural equation modeling was used report average causal mediation effects severity (National Institutes Health Stroke Scale [NIHSS]) (intravenous therapy). Results Between May 2016 December 2021, 13 641 (median age, 69 years; 50.1% women) included. Among 3002 functional data, significantly associated score (aOR, 1.16 [95% CI, 1.04–1.29]). Patients in higher neighborhoods had increased having NIHSS 1.19 1.07–1.32]). Likewise, 9.34 7.64–11.5]). neighborhood 59% mediated by (average effects: P <0.001). accounted 93% pathway discharges. association of insulin-like growth factor-i with the severity and outcomes of acute ischemic stroke
background: the aim of this study was to evaluate whether higher serum levels of insulin-like growth factor-i (igf-i) in the acute phase of ischemic stroke are associated with less severe strokes and better functional outcome in a period of 12-month follow-up. methods: from october 2014 to august 2015, patients with the diagnosis of acute ischemic stroke admitted to the stroke unit of firoozgar...
متن کاملThe Relation between GABA and L-Arginine Levels with some Stroke Risk Factors in Acute Ischemic Stroke Patients
Changes in extra and intracellular neurotransmitter amino acids concentration in the early stage of acute cerebral ischemia have been reported. In this the study, serum level of gamma aminobutyric acid (GABA) and L-Arginine in acute ischemic stroke patients was assessed. 60 patients with acute ischemic stroke and sixthy healthy volunteers as a control group were assessed. Serum GABA was measure...
متن کاملAssociation between stroke severity and fall risk among stroke patients.
BACKGROUND Falls are common after stroke. Simple, clinically practical tools are needed to easily identify patients with stroke who are at risk of falls. The objective of this study was to identify the factors associated with increased fall risk in a poststroke population. METHODS We assessed factors associated with fall risk among poststroke patients. Fall risk was determined through a histo...
متن کاملRacial and gender differences in stroke severity, outcomes, and treatment in patients with acute ischemic stroke.
BACKGROUND Previous research has indicated that women and blacks have worse outcomes after acute ischemic stroke (AIS). Little research has been done to investigate the combined influence of race and gender in the presentation, treatment, and outcome of patients with AIS. We sought to determine the association of race and gender on initial stroke severity, thrombolysis, and functional outcome a...
متن کاملSex Differences in Stroke Subtypes, Severity, Risk Factors, and Outcomes among Elderly Patients with Acute Ischemic Stroke
BACKGROUND Although the age-specific incidence and mortality of stroke is higher among men, stroke has a greater clinical effect on women. However, the sex differences in stroke among elderly patients are unknown. Therefore, we aimed to assess the sex differences in stroke among elderly stroke patients. METHODS Between 2005 and 2013, we recruited 1484 consecutive acute ischemic stroke (AIS) p...
