Spontaneous distal migration of Kuntscher intramedullary nail into leg from malunited distal femur fracture: A rare case report

Kuntscher cloverleaf nail, also called as K-nail, is an intramedullary nail (IMN), made from stainless steel with a slot along the longitudinal axis. It used in fixation of short oblique, simple transverse, Winquist- I and II comminuted fractures femur. This was case which spontaneously migrated distally, extruded completely femur into leg posterior aspect till ankle most probable cause being malunion oblique distal fracture. There were no recent reports on this type migration date. A 72-year-old male, who working farmer rural area, came to our institution pain swelling & off around left for last two months during May 2022. Local examination lower limb revealed that there ankle. palpable hard metallic object medial malleolus. single well healed surgical scar length 10 cms lateral thigh. Radiological workup done accordingly. Final diagnosis – Postoperative spontaneous malunited fracture 1/3 Treatment plan implant removal. After removal, patient discharged stable condition follow-up 6 complaints or knee joints. Distal K-nail across joint after diaphyseal rare preventable complication. recommended K-nails should be routinely removed soon radiological union consolidation confirmed, avoid postoperative complication K-nail.
