Small Abdominal Aortic Aneurysms Under Surveillance in the Over 80s: A Step Too Far?

چکیده
There remains limited evidence regarding surveillance and management of small abdominal aortic aneurysm (AAA) (males <5.5 cm women <5.0 cm) within those ≥80 years age. This review offers a unique experience 17 analysis single consultant practice in stable population (350,000), on AAA at typical District General Hospital the UK, prompted by current suggestions to consider discontinuing ≥85 age with AAA. Retrospective prospectively collected data all patients referred followed up over year period (2006 January 2023). study is subgroup analysis, age, or who became so during follow-up. Only for men index scan were included. Analysis was stratified according size time referral: < 4, 4-5, >5 <3.5, 3.5-4.5, cm.Outcomes assessed all-cause mortality, rupture, increment that reached threshold management, including treatment type. From 314 patients, 144 aged referral this (76% male, mean 81 years; range, 74-96 follow-up mean, 47.9 ± 129 months), average diameter 4.00 0.67 cm. Of 70.1% (n = 101) never intervention (23 female, 78 male); 43 did, 22 underwent elective intervention, 19 endovascular repair (2 complex) 2 open. Two emergency interventions carried out (1 1 open). Small Abdominal Aortic Aneurysms: Should We Wait?
The proper management of small abdominal aortic aneurysms (AAAs), namely those under the threshold of 5.5 cm in diameter, has been under investigation for years. Risk of rupture for this group of AAAs is higher than the rest of the population, and specific factors have been associated with increased growth rate of small AAAs. This review aims to collect and present all available research data o...
متن کاملMedical management of small abdominal aortic aneurysms.
Abdominal aortic aneurysm is a common condition that may be lethal when it is unrecognized. Current guidelines suggest repair as the aneurysm diameter reaches 5.0 to 5.5 cm. Most aortic aneurysms are detected incidentally when imaging is done for other purposes or through screening programs. Ninety percent of these aneurysms are below the threshold for intervention at the time of detection. A n...
متن کاملSurveillance strategies according to the rate of growth of small abdominal aortic aneurysms.
The management of small abdominal aortic aneurysms (AAA) is by ultrasound surveillance. The study aimed to calculate their growth rate, identify risk factors and determine appropriate screening intervals. The local screening programme and hospital records were used to identify patients with a small (< 5.5 cm) AAA. The dates and maximum diameter of serial scans of patients with two or more scans...
متن کاملPostinterventional Surveillance of Endovascular Repair of Abdominal Aortic Aneurysms
The present paper proposed to gather and underline the new developments and evidence at hand in the management of abdominal aortic aneurysms (AAA), focusing especially on the endovascular treatment (EVAR) and the surveillance of EVAR performed for AAA. The intention was to identify the existing controversies, identify a problem and give way to new research possibilities in this direction.
