Silent Brain Infarction After Endovascular Arch Procedures: Preliminary Results from the STEP Registry

چکیده
ObjectiveFew data exist concerning the rate of silent cerebral ischaemic events following endovascular treatment aortic arch. The objective this work was to quantify these lesions using STEP registry (NCT04489277).MethodsThis multicentre retrospective cohort study included consecutive patients treated with an endoprosthesis deployed in Ishimaru zone 0–3 and brain diffusion weighted magnetic resonance imaging (DW-MRI) within seven days procedure. DW-MRI performed identify location number new infarctions (SBI). All endografts were carbon dioxide flushed prior implantation.ResultsThe population 91 (mean age, 69 years; men, 64%) from two academic centres between September 2018 January 2020. procedure elective 71 (78%). for a dissection, degenerative aneurysm, or other disease 44 (49%), 34 (37%), 13 (14%) patients, respectively. Endografts 0, 1, 2 3 23 (25%), 10 (11%), 47 (52%), 11 (12%) branched fenestrated (17%), tubular (58%). At 30 days, there no deaths clinical strokes. On DW-MRI, total 245 SBI identified 45 (50%). Lesions left hemisphere 63% (153/245), predominantly middle territory (94/245). Deployment 0–1 (p = .026), placement endograft .038), proximal diameter ? 40 mm urgent .005) significantly associated presence on univariable analysis, while found be independent predictor multivariable analysis (binary logistic regression) .002).ConclusionSBI repair arch is frequent, although Innovative strategies reduce risk embolisation need developed. Few (NCT04489277). This implantation. .002).

Silent subcortical brain infarction
Introduction: The silent brain lesions detected by MRI were fairly common not only in first-ever stroke but also in normal elderly subjects. Some recent studies show the possible role of silent sub-cortical brain infarction in ischemic stroke. The aim of this study was to evaluate the frequency of silent sub-cortical brain infarction in acute first-ever ischemic stroke. Methods: In this descrip...
متن کاملEndovascular aortic arch repair after aortic arch de-branching.
PURPOSE Conventional surgical therapy of aortic arch aneurysms consists of aortic arch replacement requiring cardiopulmonary bypass and deep hypothermic circulatory arrest. This method is associated with significant morbidity and mortality, mainly due to neurologic complications and the sequelae of deep hypothermic circulatory arrest. Thus, it makes sense to work on developing less invasive sur...
متن کاملDallas Glaucoma Registry: Preliminary Results.
BACKGROUND Although glaucoma is a leading cause of blindness worldwide, yet there are no large databases where risk factors, current management options and outcomes may be evaluated. With this concept in mind, Dallas Glaucoma Registry was established to focus on an ethnically mixed North Texas population. METHODS This is a retrospective, chart review of 2,484 patients (4,839 eyes) with glauco...
متن کاملA Six-step Approach to Gain Higher Quality Results From Organotypic Hippocampal Brain Slices in a Traumatic Brain Injury Model
Background: Organotypic Hippocampal Brain Slices (OHBS) provide a better alternative to in vivo models to scrutinize Traumatic Brain Injury (TBI). We followed a well-established TBI protocol but noticed that several factors might influence the results in such a set-up. Here, we describe a structured approach to generate more comparable results and discuss why specific eligibility criteria shoul...
متن کاملEmerging spectra of silent brain infarction.
S ilent brain infarction (SBI) is an increasingly recognized, and yet still poorly understood, clinical disorder characterized by the often incidental finding of cerebral infarction on imaging in the absence of clinically apparent neurologi-cal deficit. Although first described ≈50 years ago by Fisher 1 through autopsy studies, it was not until the development of more sensitive imaging modaliti...
