Pseudo hypobicarbonatemia is a rare phenomenon associated with spuriously low bicarbonate levels in the presence of elevated triglycerides (TG). TG’s interfere commonly used enzymatic assays basic metabolic panel (BMP), giving falsely levels. It adds to diagnostic dilemma and possible extensive workup identify cause. Previous case reports have plasmapheresis these settings. We report successful use insulin infusion managing pseudo-acidosis secondary severely high TG Our patient was 53-year-old female who presented complaints progressive fatigue.
