Aims • Ensure compliance with seclusion trust policy and guidelines by the mental health team (goal of 100 percent). Confirm that proper documentation commencement, periodic reviews, termination is always maintained (goal: Methods Retrospective collection data from one adult male Psychiatric Intensive Care unit female ward. Our sample consisted patients who were secluded between time period September 2021 June 2022. 33 episodes met this inclusion criteria. Data collected OpenRio progress notes section. We developed a tool for monitoring reviews included different about demographics other variables in reviews. Results found out following: In regard to demographics, predominant age groups are 20 40 years old, although there also an increase number people 50 70 old ethnicity was white British. The rationale start continuity documented 100% cases our episodes. initial medical review completed first hour 81.82% (27) , 18.18% (6) cases, it not within window. 4 doctor contacted meet one-hour limit. 2 episodes, reasons being late documented. hourly nursing 93.94% (31). There 6.06 % (2 episodes) completed. No specific reason missed (before MDT / consultant reviews) 24 9 when window hourly. 5 patient sleeping, so didn't contact doctor. no delay. However, extra 1-2 hours duration time. 8 26 (78.79%). 7 primary episode started on weekend afternoon or early evening after normal working day conducted next day. Two daily – at least responsible clinician (following review) : 3 (9.09%), two without documentation. Rational continue/ end each completed: 32 continue where ended doctors. Physical observations record :100% physical record. Conclusion Recommendations: Increase awareness importance completing conducting teaching sessions local academic program informal staff. Adding junior doctors handbook discuss during induction meetings. Allocate flyers posters information stations office. Completing re-audit cycle gauge scope change.
