Reversible ethambutol-induced optic neuropathy: report of a rare case

BackgroundEthambutol (EMB) is one of the first-line anti-tuberculosis therapy. One its precarious side effects ethambutol-induced optic neuropathy (EON). The ocular manifestations EON include painless loss central vision and cecocentral scotomas in visual field. Case DescriptionA 60-year old man presented with gradual since 3 months prior to visit. accompanying symptoms were frequent headache without double photophobia. He had been diagnosed pulmonary tuberculosis (TB) for 9 consumed EMB 7 before being advised discontinue. There was neither history systemic diseases nor family neuropathy. Examination showed reduced acuity positive relative afferent pupillary defect (RAPD) on right eye. Humphrey test bilateral generalized loss. Ocular computed tomography (OCT) retinal nerve fiber layer (RNFL) thickness within normal limit. Magnetic resonance Imaging (MRI) brain indicated unremarkable result Based examinations listed above, this patient as stopped immediately. Patient given oral citicoline 1000mg zinc supplementation month. After 1 st rd month follow up, patient’s function gradually improved. ophthalmic recovery function. ConclusionsThis case suggested that an early detection intervention has promising outcome. a reversible if toxicity monitored closely among patients who are prescribed regiment.
