The review presents literature data on the problem of respiratory muscle (RM) dysfunction in patients with chronic obstructive pulmonary disease (COPD). main features their pathophysiological interactions and clinical practice values were observed. Both local systemic factors pathogenesis COPD are important development RM dysfunction. Local associated lung remodeling need to overcome excessive breathing resistance, which increases intensity work, contributes hypertrophy insufficiency. Chronic inflammation, oxidative stress, proteolysis other manifestations change metabolism, structural functional organization RM. severity morpho-functional disorders metabolic status depends gravity emerges early stages as myocytes’ hypertrophy, increasing capillarization mitochondrion density. At late it appears diffuse replacement fibers by connective tissue. Low nutritional status, protein-energy deficiency, electrolyte imbalance, endocrine background progressive insufficiency tissue hypoxia contribute weakness. combination COPD, acute cerebral circulation heart failure sharply worsens function prognosis comorbid diseases. Also, having is significantly aggravated at age senility. discusses possibilities pharmacological correction aspects this problem.
