چکیده

Background: Decompressive craniectomy is an effective treatment for malignant middle cerebral artery (MCA) territory infarct. But the age limit surgery not well defined. The time window of considered less than 96 hours. Methods: This retrospective study 60 patients MCA infarct who underwent decompressive selected in this and using modified Rankin score. Results: In 36 were years above years. 38 right sided 22 left Out patients, operated within 48 hours onset symptoms between to hours. 49% had score (mRS) 0–3 at 3 months follow up while only 37% after mRS up.Conclusions: infarction can reduce case fatality rate but favorable outcome questionable. Hence surgical decision should depend on social patient factors.

A case of complete middle cerebral artery territory infarction associated with some haemorrhagic transformation at 36 hours post onset, and at one day after decompressive surgery. Note the effect of surgery on midline shift and ventricular compression. The patient survived with a Modifi ed Rankin Score of 3. But would the patient have done as well without surgery? Here we present the two opposi...

Decompressive craniectomy for the treatment of malignant infarction of the middle cerebral artery

Early decompressive craniectomy (DC) has been shown to reduce mortality in malignant middle cerebral artery (MCA) infarction, whereas efficacy of DC on functional outcome is inconclusive. Here, we performed a meta-analysis to estimate the effects of DC on malignant MCA infarction and investigated whether age of patients and timing of surgery influenced the efficacy. We systematically searched P...

Quality of life after decompressive craniectomy for malignant middle cerebral artery infarction.

Malignant middle cerebral artery (MCA) infarction is a devastating condition leading to early death in nearly 80% of cases due to the rapid rise of intracranial pressure despite maximum medical management of the ischaemic brain oedema. Decompressive craniectomy (DC) has been proposed to prevent brain herniation in malignant MCA infarction, but it remains controversial in the absence of randomis...
