Race disparities in atrial fibrillation detection using implantable loop recorder for cryptogenic stroke

چکیده
Abstract Background Cryptogenic stroke represents about 25–30% of all ischemic strokes. Continuous electrocardiographic monitoring using implantable loop recorder (ILR) can detect asymptomatic and undiagnosed atrial fibrillation (AF). Purpose To assess patient-specific factors in ILR implantation AF detection patients with cryptogenic Methods We retrospectively reviewed 379 indicated for implantation, from January 2017 to June 2021 at our university hospital. evaluated patient demographic clinical characteristics the risk associated higher incidence detection, logistic regression models. Results Of ILRs implanted, 60.4% were males 71.2% Caucasian, mean age 67.6 years, BMI 28.5. The CHADSVASC score 3.9 time stroke. These had prior diagnosis diabetes mellitus (27.4%), hypertension (65.7%), chronic kidney disease (19.5%), coronary artery (20%) congestive heart failure (2%). entire cohort, was diagnosed 16% 14 months follow up. There more females on (20% v. 18.7%, p>0.05). Caucasians likely have detected compared non-Caucasians (OR 2.3, 95% CI, p<0.02). 52 mobile cardiac outpatient telemetry (MCOT) before having implantation. not MCOT, but 15% monitoring. In univariate analysis, increasing is strongly (p=0.03). Conclusions continues exist race disparities eventual leading further socioeconomic health disparities. should be considered especially elderly given detection. Insertable cardiac event recorder in detection of atrial fibrillation after cryptogenic stroke: an audit report.
BACKGROUND AND PURPOSE Atrial fibrillation (AF) is the most frequent risk factor in ischemic stroke but often remains undetected. We analyzed the value of insertable cardiac event recorder in detection of AF in a 1-year cohort of patients with cryptogenic ischemic stroke. METHODS All patients with cryptogenic stroke and eligibility for oral anticoagulation were offered the insertion of a card...

Ischemic stroke is an important cause of morbidity and mortality when untreated. Identifying atrial fibrillation is important because atrial fibrillation ischemic related strokes are associated with an increased risk of disability and death compared with strokes of other etiologies and tend to recur without anticoagulation. However, atrial fibrillation detection can be difficult when it is asym...
متن کاملParoxysmal atrial fibrillation in cryptogenic stroke
INTRODUCTION Paroxysmal atrial fibrillation (PAF) is perhaps the most underdiagnosed mechanism of apparent cryptogenic stroke (CS). Various studies have shown that increasing the duration of monitoring can increase the diagnosis of PAF in CS. METHODS We compared demographic and risk factors for ischemic stroke across different TOAST (Trial of ORG 10172 in Acute Stroke Treatment) stroke subtyp...
متن کاملAtrial fibrillation in patients with cryptogenic stroke.
BACKGROUND Atrial fibrillation is a leading preventable cause of recurrent stroke for which early detection and treatment are critical. However, paroxysmal atrial fibrillation is often asymptomatic and likely to go undetected and untreated in the routine care of patients with ischemic stroke or transient ischemic attack (TIA). METHODS We randomly assigned 572 patients 55 years of age or older...
