Pulmonary Artery Aneurysm Associated With Aspergilloma in a Patient Diagnosed With Granulomatosis With Polyangiitis

Granulomatosis with polyangiitis (GPA) is an autoimmune, necrotizing granulomatous disease that affects small- and medium-sized blood vessels. Aspergilloma a fungal mass of Aspergillus usually found in the preexisting cavity lung parenchyma. Surgical resection mainstay treatment aspergilloma. In this article, we present case 70-year-old male GPA aspergilloma who presented massive, life-threatening hemoptysis. Further workup chest computed tomography (CT) pulmonary angiogram demonstrated artery pseudoaneurysm along wall which was emergently managed embolization required monitoring medical intensive care unit. This report alerts clinicians to maintain high level suspicion for aneurysm if degree hemoptysis higher than expected.
