Preventing Arrhythmic Death in Patients With Tetralogy of Fallot

Patients with tetralogy of Fallot are at risk for ventricular arrhythmias and sudden cardiac death. These abnormalities associated pulmonary regurgitation, right enlargement, a substrate discrete, slowly-conducting isthmuses. Although these arrhythmic events rare, their prediction is challenging. This review will address contemporary assessment prevention strategies. Numerous variables have been proposed to predict who would benefit from an implantable cardioverter-defibrillator. Current stratification models combine independently factors into scores. Cardiac magnetic resonance imaging, QRS fragmentation assessment, electrophysiology testing in selected patients may refine some models. Interaction between left function emerging as critical factor our understanding disease progression assessment. Multicenter studies evaluating mitigating strategies such valve replacement, ablative strategies, use cardiac-defibrillators needed moving forward.
