Press-fit vs. cemented humeral stem fixation for reverse shoulder arthroplasty: functional outcomes at a mean follow-up of 9.5 years

چکیده
BackgroundReverse shoulder arthroplasty (RSA) is a reliable, long-term treatment option for degenerative pathology. However, the functional outcomes degrade around fifth year postoperation, particularly internal and external rotation. Long-term radiographic changes have been reported humeral stem, depending on fixation method.ObjectivesThe primary objective was to compare clinical of RSAs by method—cemented vs. press-fit—after minimum follow-up 5 years. We hypothesized that proximal osteolysis impacts outcomes.Design MethodsOur study compared 2 matched cohorts: 56 with cemented stems press-fit stems. evaluated all patients using Constant-Murley score (CMS) range motion (ROM), which we measured preoperatively at final visit. radiographs from visit look tuberosity resorption, signs stress shielding, periprosthetic radiolucent lines, scapular notching.ResultsAt mean 9.5 years (5-21), regardless method, RSA procedure helped improve CMS active joint ROM postoperatively, except rotation elbow side in group. Humeral-sided Radiographic Changes Following Reverse Total Shoulder Arthroplasty
Background: We sought to characterize humeral-sided radiographic changes at a minimum of 2 years after reverseshoulder arthroplasty (RSA) to determine their association with specific implantation techniques. Methods: The immediate and most recent postoperative anteroposterior radiographs of 120 shoulders with primary RSAand a minimum of 2-years of radiographic follow-up were an...
متن کاملFunctional outcomes of reverse shoulder arthroplasty compared with hemiarthroplasty for acute proximal humeral fractures.
BACKGROUND Complex acute proximal humeral fractures may require prosthetic replacement of the proximal humerus. Reverse shoulder arthroplasty (RSA) has been suggested as an alternative to hemiarthroplasty in the management of such fractures. This study compared the functional outcomes of RSA with hemiarthroplasty in patients with acute proximal humeral fractures. MATERIALS AND METHODS All pat...
متن کاملReverse shoulder arthroplasty for proximal humerus fracture using a dedicated stem: radiological outcomes at a minimum 2 years of follow-up—case series
BACKGROUND Complex proximal humeral fractures are very difficult to treat particularly in patients older than 65 years with an osteoporotic bone and tuberosities compromised. The goal of this paper is to evaluate radiological outcomes at mid-term follow-up of proximal humerus fractures treated with reverse shoulder arthroplasty using a dedicated fracture stem. MATERIALS AND METHODS The study ...
متن کاملReverse shoulder arthroplasty with a cementless short metaphyseal humeral implant without a stem: clinical and radiologic outcomes in prospective 2- to 7-year follow-up study.
BACKGROUND Reverse shoulder prostheses are increasingly used in recent years for treatment of glenohumeral arthropathy with deficient rotator cuff. Bone preservation is becoming a major goal in shoulder replacement surgery. Metaphyseal humeral components without a stem were developed to minimize bone resection and preserve bone. This study evaluated the clinical and radiologic outcomes at 2 to ...
متن کاملFemoral press-fit fixation in ACL reconstruction using bone-patellar tendon-bone autograft: results at 15 years follow-up
BACKGROUND If anterior cruciate ligament (ACL) reconstruction is to be performed, decision regarding graft choice and its fixation remains one of the most controversial. Multiple techniques for ACL reconstruction are available. To avoid disadvantages related to fixation devices, a hardware-free, press-fit ACL reconstruction technique was developed. The aim of this study was to evaluate clinical...
