Predictors of Suboptimal Coronary Blood Flow during Primary Percutaneous Intervention

Background and aim: Suboptimal coronary blood flow following first percutaneous intervention (PCI) is a multivariate phenomenon with several causes. Despite substantial research, recognized modifiable risk factors an effective management plan are lacking. The present study intended to determine the causes of suboptimal during primary intervention. Patients Method: A retrospective was carried out on 486 STEMI patients in Interventional Cardiology Department, MTI-Hayatabad Medical Complex, Peshawar from June 2021 May 2022. (age>20 years) presented initial within 12 hours after onset symptoms were enrolled. Patients’ physical examination, medical history, different factors, vital signs assessment, echocardiography, Killip class, laboratory investigation, post-procedural ECG recorded along PCI data associated catheterization. Clinical outcomes included MACE, ischemia-driven target vessel revascularization, reinfection, cardiac mortality, post-catheterization recorded. Descriptive statistics done SPSS version 26. Results: Of total patients, there 412 (84.8%) male 74 (15.2%) females. overall mean age 52.62± 8.64 years. incidence (TIMI ≤2) optimal (TIMI-3 flow) 112 (23%) 374 (77%) respectively. Age [OR 1.041/year: p<0.001], stent length 1.019 per 1 mm], low SBP 1.021 mm Hg], thrombus burden grade 1.78], baseline TIMI (≤1) 1.72; p=0.021] independent predictors. Cardiac mortality MACE significantly higher 30-days as compared flow. prevalence in-hospital 9.1% 2.1% Conclusion: found that substantially cardiovascular STEMI. most significant predictor poor predilatation prior stenting.
