POS1160 BRUCELLAR SPONDYLODISCITIS: CLINICAL AND MAGNETIC RESONANCE IMAGING FINDINGS

Background: Osteoarticular involvement is one of the most common complications brucellosis and it can occur in up to two thirds patients infected. Vertebral during considered not only as prevalent localization but also severe one. Objectives: The aim present study was describe clinical magnetic resonance imaging features brucellar spondylodiscitis. Methods: We conducted a retrospective including 21 who have been hospitalized for spinal from 1998 2019. diagnosis based on symptoms, MRI findings isolation brucella species blood or tissue specimens and/or positive Wright agglutination test. Results: Twenty-one were included. mean age 57.5 years [33-74] (13 males 8 female). geographical origin rural 76% cases. median duration symptoms progression before 4.5 months [1-8].The main symptom leading seek medical care an inflammatory back pain found all It associated with unilateral radiculopathy cases bilateral 1 case. other fever 17 patients, sweats 14 cases, weight loss 10 hepatomegaly patient. physical examination revealed tenderness palpation 12 motor weakness lower limbs 2 Brucella test ≥1/160 Blood cultures negative erythrocyte sedimentation rate (ESR) serum C-reactive protein level 40 mm 11mg/dl) respectively. Spondylodiscitis located lumbar dorsal cervical spine respectively 10, 3 affected L4-L5 5 followed by T10-T11 more than vertebrae 4 Associated sacroiliitis All images showed hypo intense signal T1 weighted image hyper T2 image. Disc space narrowing body osteolysis Epidural collection documented size 5cm. Paravertebral peri-vertebral abscesses detected Intradiscal observed Cord compression root nerve noted Biopsy performed bacteriological contributory received antibiotic treatment combination three drugs, corticosteroids prescribed patient underwent surgical intervention. evolution good 16 suffered chronic pain, had persistent neuropathic paresis.
