چکیده
Background: The main consequence of osteoporosis is the occurrence fractures due to bone fragility, with important sequelae in terms disability and mortality. It has been already demonstrated that information about mass density (BMD) alone not sufficient predict risk fragility fractures, since several occur patients normal BMD [1]. Fragility Score a parameter allows estimate skeletal thanks trans-abdominal ultrasound scan performed Radiofrequency Echographic Multi Spectrometry (REMS) technology. calculated by comparing results spectral analysis patient’s raw signals reference models representative fragile non-fragile bones [2]. dimensionless parameter, which can vary from 0 100, proportion degree independently BMD. Objectives: This study aims evaluate effectiveness Score, measured during densitometry exam REMS technology at lumbar spine, identifying incident osteoporotic follow-up period 5 years. Methods: Caucasian women age between 30 90 were scanned spinal DXA. incidence was assessed ability discriminate without subsequently evaluated compared discriminatory T-score DXA REMS. Results: Overall, 533 (median age: 60 years; interquartile range [IQR]: 54-66 years) completed 42 months; IQR: 35-56 months), 73 had sustained an fracture. Both median values significantly lower fractured than for non-fractured ones, conversely, higher (Table 1). Table 1. Analysis Median ranges (IQR) are reported. p-value derived Mann-Whitney test. Patients fracture [median (IQR)] -1.9 (-2.7 -1.0) -2.6 (-3.3 -1.7) 0.0001 -2.0 (-2.8 -1.1) -2.7 (-3.5 -1.9) <0.0001 29.9 (25.7 36.2) 53.0 (34.2 62.5) By evaluating capability with/without obtained value ROC area under curve (AUC) 0.80, AUC (0.66) (0.64), difference statistically significant (Figure Figure comparison classification fractures. Furthermore, correlation low, Pearson coefficient r =-0.19 -0.18 T-score. Conclusion: found be effective tool prediction population women, performances superior those values. Therefore, this presents high potential as diagnostic early identification subsequent treatment fragility. References: [1]Diez Perez A et al. Aging Clin Exp Res 2019; 31(10):1375-1389. Hip and fragility fracture prediction by 4-item clinical risk score and mobile heel BMD: a women cohort study
BACKGROUND One in four Swedish women suffers a hip fracture yielding high morbidity and mortality. We wanted to revalidate a 4-item clinical risk score and evaluate a portable heel bone mineral density (BMD) technique regarding hip and fragility fracture risk among elderly women. METHODS In a population-based prospective cohort study we used clinical risk factors from a baseline questionnaire...
متن کاملClinical Utility of Using Lumbar Spine Trabecular Bone Score to Adjust Fracture Probability: The Manitoba BMD Cohort.
Decreased lumbar spine trabecular bone score (TBS), a dual-energy X-ray absorptiometry (DXA)-derived image texture measurement, is a risk factor for major osteoporotic fracture (MOF) and hip fracture (HF) independent of 10-year fracture probability estimated using FRAX. We determined how often applying the TBS adjustment to fracture probability altered treatment qualification. Using a populatio...
متن کاملA mental-functional risk score for prediction of hip fracture.
We examined the apparent value of questions of functional status and mental score in determining the risk of hip fracture from a large retrospective case--control study undertaken in six countries. Of nine questions utilized in 75% of the MEDOS study population, four questions were found to be statistically significant and independently related to the risk of hip fracture. From this an additive...
متن کاملInterobserver discrepancies in distance measurements from lumbar spine CT scans.
Lumbar spine computed tomographic (CT) scans of 10 patients were examined independently at two levels by five experienced radiologists. At each level the minimum midline sagittal diameter was measured, and at each intervertebral space the left foramen was measured for its minimum diameter. Statistically significant differences were found between the measurements of different observers, differen...
متن کاملRisk assessment of fragility fracture: NICE guideline.
British Journal of General Practice, December 2012 667 INTRODUCTION Fragility fractures result from mechanical forces that would not ordinarily result in fracture. The World Health Organization (WHO) has quantified this as forces equivalent to a fall from a standing height or less. Reduced bone density is a major risk factor for fragility fractures. The prevalence of osteoporosis increases mark...
