POS0103 IN RHEUMATOID ARTHRITIS, THE ASSOCIATION BETWEEN ANTI-MODIFIED PROTEIN ANTIBODIES AND LONG-TERM OUTCOMES IS DOMINATED BY THE EFFECT OF ANTI-CITRULLINATED PROTEIN ANTIBODIES

چکیده
Background In rheumatoid arthritis (RA) many patients harbor autoantibodies against post translationally modified proteins (anti-modified protein antibodies (AMPA)): anti-citrullinated (ACPA), anti-carbamylated (anti-CarP) and anti-acetylated (AAPA). All three AMPA have been described to be associated with an increase in radiographic joint damage ACPA also a lower chance of obtaining sustained drugfree remission (SDFR). However, it is unclear whether the effects these are independent each other, testing for all offers additional information regarding clinical outcomes. Objectives To investigate individual combined associations radiological progression SDFR RA. Methods 612 RA from Leiden Early Arthritis Clinic, we measured IgG anti-CCP2-assays, anti-CarP using homocitrullinated native (as control) fetal calf serum, AAPA by means novel assays consisting cyclic peptides either acetylated or norleucine residues control). Sharp-van der Heijde scores (SHS) were determined 2685 sets radiographs yearly intervals 7 years. The association SHS was assessed multivariate normal regression model. defined as absence synovitis after discontinuation DMARD treatment. autoantibody-status Kaplan-Meier curves. very small number single-positive (n=4, 1%) precluded analyses this subgroup. Results prevalence shown Table 1. A higher found compared without antibodies, triple-positive versus (Figure 1A, p=0.04). if due specific autoantibody, stratified AMPAs. Interestingly, no difference between ACPA-positive strata 1B). ACPA-negative stratum, significant zero solely (p=0.02). Moreover less triple-antibody-positive than single-antibody-positive achieved (P<0.001), Figure 1C. After stratification AMPA, percentage strata, 1D. Conclusion patients, presence other influences neither progression, nor SDFR. Thus, long-term phenotype particularly dependent on ACPA, AMPA. Therefore, there appears added value predicting outcome patients. Disclosure Interests None declared

Avidity maturation of anti-citrullinated protein antibodies in rheumatoid arthritis.
OBJECTIVE Anti-citrullinated protein antibodies (ACPAs) are highly specific for rheumatoid arthritis (RA) and are present years before the onset of symptoms. The avidity of autoantibodies can have a strong impact on their effector potency. This study was undertaken to analyze the avidity of ACPAs in serum samples obtained from ACPA-positive healthy individuals (predisease), patients with early ...
متن کاملComparative Utility of Anti-Citrullinated Protein Antibodies Verses Rheumatoid Factor in the Management of Rheumatoid Arthritis
The existence of autoantibodies against citrullinated proteins was first described in the 1970s in the patients with rheumatoid arthritis. This time the biochemical basis of antibody reactivity against antigens called keratin, and filaggrin was investigated. It is recognized now that during several natural processes such as apoptosis and some inflammatory processes, an enzyme called Peptidyl ar...

متن کاملThe role of anti-citrullinated protein antibodies (ACPA) in the pathogenesis of rheumatoid arthritis
The most specific autoimmunity known for rheumatoid arthritis (RA) is reflected by generation of anti-citrullinated protein antibodies (ACPA). Presence of ACPA in established RA is associated with disease severity, while generation of ACPA at early developmental phases of RA can have a strong predictive value for progressing to the full-blown disease. Hence, development of ACPA may be of crucia...
متن کاملCorrelation between anti-nuclear antibodies (ANA), rheumatoid factor (RF), anti-cyclic citrullinated peptides antibodies (Anti-CCP) and percentage and type of pulmonary involvement in patients with rheumatoid arthritis, Booali Hospital Tehran (2016-2
Background: Controlling the pulmonary involvement, as one of the most common extra-articular consequences in rheumatoid arthritis (RA), can improve the life quality and survival rate of patients. This study aimed to evaluate the correlation between disease-related inflammatory factors and the severity and type of lung involvement in patients, which is a practical step toward early detection and...
