چکیده
Aberrant conduction during orthodromic reciprocating tachycardia (ORT) can prolong the ventriculo-atrial time, which is found essential for reentry in certain situations. To investigate prevalence and different mechanism of aberrancy-dependent ORT. ORT cases relying on aberrancy from 220 our center were searched using following criteria: (1) Multiple episodes with aberrant ECG ; (2) No baseline BBB sinus rhythm; (3) Absence narrow QRS form unless isoproterenol used; (4) Restoration bundle branch giving rise to termination or circuit alteration. ECGs intracardiac tracings study carefully examined. 3 patients (1.4%) showed phenomenon dependence aberrancy. Clinical short time. All accessory pathways (APs) located at anterolateral regions atrioventricular annulus. None them had block. Creating functional block was necessary induce tachycardias. In first patient left (LBBB), a late-delivered premature ventricular contraction (PVC) able advance next A reset tachycardia, diagnostic Meanwhile, restored narrowed complex. Tachycardia then terminated by advanced wavefront reaching insertion site AP refractory. second case, always preceded spontaneous LBBB restoration, V subsequent MA advanced, suggesting anterograde AV node. other case was, induction required both right slow pathway conduction. When given, fast recovered followed made limb again switched pathway. successfully ablated procedure. have been free arrhythmias least 6 months. Extra transseptal time caused be an integral part ORT, explain infrequent late onset unsustainable useful understanding localizing

Physiological substrate for antidromic reciprocating tachycardia. Prerequisite characteristics of the accessory pathway and atrioventricular conduction system.
BACKGROUND Although the anatomic "substrate" for the occurrence of antidromic reciprocating tachycardia (ART) has been previously examined, the underlying physiological substrate for this unusual arrhythmia in patients with the Wolff-Parkinson-White syndrome has not been thoroughly characterized. METHODS AND RESULTS The electrophysiological properties of the accessory pathway and normal ventr...
متن کاملDifferentiation of atypical atrioventricular node re-entrant tachycardia from orthodromic reciprocating tachycardia using a septal accessory pathway by the response to ventricular pacing.
OBJECTIVES The purpose of this study was to determine whether the response to ventricular pacing during tachycardia is useful for differentiating atypical atrioventricular node re-entrant tachycardia (AVNRT) from orthodromic reciprocating tachycardia (ORT) using a septal accessory pathway. BACKGROUND Although it is usually possible to differentiate atypical AVNRT from ORT using a septal acces...
متن کاملPATHOPHYSIOLOGY AND NATURAL HISTORY PREEXCITATION Preexcited reciprocating tachycardia in patients with Wolff-Parkinson-White syndrome: incidence and mechanisms
During the electrophysiologic study of 435 patients referred for evaluation of WolffParkinson-White syndrome, 42 (10%) had preexcited reciprocating tachycardia (defined as a macroreentrant tachycardia that used an accessory atrioventricular [AV] pathway for antegrade conduction). The ages of the patients ranged from 9 to 67 years (27 + 14). Thirty-three were male patients, nine female, and eigh...
متن کاملEnhanced atrioventricular conduction in patients without preexcitation syndrome: relation to heart rate in paroxysmal reciprocating tachycardia.
We studied the electrophysiologic characteristics of atrioventricular (AV) nodal conduction in patients with reciprocating tachycardia (RT) without ventricular preexcitation, and the relation of these characteristics to RT cycle length (CL). Thirty-five symptomatic patients who had a normal PR interval (0.13-0.20 second) during sinus rhythm underwent detailed intracardiac electrophysiologic stu...
متن کاملA technique for the rapid diagnosis of atrial tachycardia in the electrophysiology laboratory.
OBJECTIVE The purpose of this study was to determine if the atrial response upon cessation of ventricular pacing associated with 1:1 ventriculoatrial conduction during paroxysmal supraventricular tachycardia is a useful diagnostic maneuver in the electrophysiology laboratory. BACKGROUND Despite various maneuvers, it can be difficult to differentiate atrial tachycardia from other forms of paro...
