چکیده
The location of conduction abnormality including activation slowing and wavefront discontinuity, which often corresponds to the critical zones reentrant circuit in scar-mediated ventricular tachycardia (VT), is affected by direction during substrate mapping. We hypothesized that an abrupt change paced-QRS morphologies may occur at region with abnormality. This study assessed velocity mapping least two rhythms where was obtained. In a retrospective review 57 patients who underwent functional for VT, 21 it multiple rhythms, intrinsic rhythm paced high-density pace-mapping area interest. 10 these patients, those sites, as determined direct termination ablation (4 VTs, 33%) or findings (8 67%) were identified total 12 VTs. Conduction velocities isthmus based on difference local time between sites along site scores changed abruptly. Pace-mapping scores, obtained adjacent presumed isthmus, showed 92% [interquartile range [IQR] 89-96%] exit side 48% [IQR 41-56%] entrance side. Ten (71±9 years, 9 (90%) male, 6 (60%) nonischemic) sinus (n=7) right pacing (n=5) left (n=7). pacemap 0.37m/sec 0.19-0.43 m/sec], no slower than 0.25 m/sec, defined previous studies. However, faster one significantly slowed down when changing other (0.41 m/sec 0.25-1.05 m/sec] vs. 0.09 [IQR0.07-0.22 P<0.01) (Fig.1). morphology novel strategy, shown independent mapping, can identify high-yield regions being reentry.

Functional Pace-Mapping Responses for Identification of Targets for Catheter Ablation of Scar-Mediated Ventricular Tachycardia
Background—Myocardial scars harbor areas of slow conduction and display abnormal electrograms. Pace-mapping at these sites can generate a 12-lead ECG morphological match to a targeted ventricular tachycardia (VT), and in some instances, multiple exit morphologies can result. At times, this can also result in the initiation of VT, termed pace-mapped induction (PMI). We hypothesized that in patie...
متن کاملTransmural "Scar-to-Scar" Reentrant Ventricular Tachycardia
We describe a scar-related reentrant ventricular tachycardia circuit with a proximal segment in an endocardial basal septal scar and an exit in a region of slow conduction in a non-overlapping region of epicardial basal lateral scar. The 12-lead EKG demonstrates criteria for a basal lateral epicardial VT, however the same morphology could be produced with a longer stim-latency with pace mapping...
متن کاملHigh-Density Mapping in Ventricular Tachycardia Ablation: A PentaRay® Study
Background High-density mapping of ventricular tachycardia (VT) with PentaRay® (Biosense-Webster) provides high resolution with discrimination of local abnormal electrograms and slow conducting channels. We evaluate the feasibility of PentaRay® to characterize the anatomical substrate and assume an influence of the outcome despite limitations. Methods Over a 24-month period, 26 endocardial an...
متن کاملA Prospective Study of Ripple Mapping the Post-Infarct Ventricular Scar to Guide Substrate Ablation for Ventricular Tachycardia.
BACKGROUND Post-infarct ventricular tachycardia is associated with channels of surviving myocardium within scar characterized by fractionated and low-amplitude signals usually occurring late during sinus rhythm. Conventional automated algorithms for 3-dimensional electro-anatomic mapping cannot differentiate the delayed local signal of conduction within the scar from the initial far-field signa...
