PO-02-083 LOCATION OF LEADLESS PACEMAKER IMPLANT AND PACING-INDUCED CARDIOMYOPATHY

چکیده
Leadless pacing (LP) is associated with a lower incidence of pacing-induced cardiomyopathy (PICM) compared to transvenous pacing. However, it unclear if the location implantation LP makes difference in PICM. To compare PICM based on 276 patients [Men: 157 (57%), Women: 119 (43%), mean age: 73 ± 11 yrs, all EF > 50%] who received (Micra) between January 2019 – June 2022 formed study cohort. AV micra and VR were implanted 93 (34%) 183 (67%) respectively. Fluoroscopically, Interventricular septum was divided into 3 equal thirds [apical (AS), mid (MS) high (HS)]. During follow-up, defined as an left ventricular ejection fraction (EF) drop ≥ 10%. 94/276 (34%), 123/276 (45%), 59/276 (21%) had at AS, MS HS locations 8 5 months 23 (8.3%) developed (Men: 9/23 (39%), women: 14/23 (61%) (P = 0.023). Of them, 11/94 (12%), 8/123 (7%) 4/59 0.057) implantation. AS (11/94 (12%) higher risk MS+HS [12/182 0.044)]. The hazard [Adjusted Hazard ratio: 1.76 (1.13 - 3.25) P 0.044, (adjusted for age, gender, percentage burden EF)]. Incidence low (8.3%). Women are men. placement placement. Larger randomized studies needed confirm our findings.

DOI: 10.5935/abc.20160144 67 year-old female patient, pacemaker user since 1983 due to complete atrioventricular block. Between 1991 and 2004, the patient underwent six surgeries. The first was to replace the batteries, and the others for electrode displacement, generator extrusion, and finally for endocarditis, when surgical extraction of the system and epicardial electrode implantation in the...
متن کاملLeadless cardiac pacing
Transvenous cardiac pacemakers definitely improve quality of life and reduce mortality in at-risk patients, but they are associated with several potential device-related complications. Approximately 10% of patients experience complications related to transvenous implantation of the pacemaker. These may be attributable to either the pulse generator (hematoma, skin breakdown, pocket infection) or...
متن کاملPacemaker-induced takotsubo cardiomyopathy
Introduction Takotsubo cardiomyopathy (TCM) is a form of acute cardiomyopathy showing left ventricular apical ballooning. Whilst the exact mechanism of TCM remains unknown, it is hypothesized that an excess of circulatory catecholamine's to an exaggerated sympathetic from an intense physical or emotional distress as the cause. We report the rare case of a 76-year-old woman who developed TCM aft...
متن کاملRescue Leadless Pacemaker Implantation in a Pacemaker-Dependent Patient with Congenital Heart Disease and no Alternative Routes for Pacing.
Congenital heart disease patients are considered a unique group of patients regarding their high risk of conduction abnormalities , whether de novo or surgically induced , and the challenges in both implantation and management of device related complications. We present a case of a pacemaker-dependent patient with congenital heart disease who experienced complications of both previous epicardia...
متن کاملPacing-Induced Dilated Cardiomyopathy
Background. The right ventricle (RV) receives part of its systolic pumping force from the left ventricle through systolic ventricular interaction. The purpose of this study was to determine the effects of dilated cardiomyopathy on left ventricular to right ventricular (LV-to-RV) systolic interaction. Methods and Results. Studies were performed in six normal pigs and in six pigs in which dilated...
