Pigmentierte villonoduläre Synovitis

Zusammenfassung Ziel In diesem Beitrag wird evaluiert, ob die radiologischen Diagnosekriterien ausreichen, um Diagnose einer pigmentierten villonodulären Synovitis (PVNS) anhand eines Magnetresonanztomographie(MRT)-Befunds sicher stellen zu können und daraus eine Operationsindikation abzuleiten. Hierzu wurde überprüft, inwieweit radiologische mit dem histopathologischen Befund übereinstimmte. Methoden retrospektiven Datenanalyse wurden 52 Patienten PVNS (22 männlich, 30 weiblich, mittleres Alter: 38 Jahre), sich zwischen 1991 2019 einem operativen Eingriff unterzogen hatten, analysiert. Einschlusskriterien waren ein positiver MRT-Befund Hinweis auf Hämosiderin anschließende Synovektomie sowie entsprechend dokumentierte histopathologische Aufarbeitung. Ergebnisse 71 % der Fälle stimmten das histologische Ergebnis überein, was jedoch darauf hindeutet, dass nach radikalen in etwa 29 histologisch keine nachgewiesen werden konnte. Das Kniegelenk stellte als am häufigsten betroffene Gelenk dar, gefolgt von Hüft- Sprunggelenk. Diskussion Die tritt meist bei jungen Erwachsen auf. Frauen im eigenen Kollektiv etwas häufiger betroffen Männer. aktuellen Literatur findet man keinen etablierten Algorithmus zur Behandlung PVNS. Im Zweifelsfall sollte unbedingt Diagnosesicherung mittels arthroskopischer oder offener Biopsiegewinnung vorgeschaltet werden. Schlussfolgerung Aufgrund Invasivität ist präoperative Biopsie weitere Möglichkeit eingesetzt Eine enge interdisziplinäre Zusammenarbeit Radiologen Tumorradiologen hierbei essenziell.
