Perinatal Outcomes of Small for Gestational Age in Twin Pregnancies: Twin vs. Singleton Charts

چکیده

Twin pregnancies are commonly assessed using singleton growth and birth weight reference charts. This practice has led to a significant number of twins labelled as small for gestational age (SGA), causing unnecessary interventions increased risk iatrogenic preterm birth. However, the use twin-specific charts remains controversial. study aims assess whether estimated fetal (EFW) (BW) more predictive adverse outcomes compared Centiles EFW BW were calculated previously published twin Categorical data Chi-square or McNemar tests. The included 1740 pregnancies, with following perinatal recorded: death, <34 weeks, hypertensive disorders pregnancy (HDP) admissions neonatal unit (NNU). Twin-specific identified prenatally postnatally smaller proportion infants SGA showed higher percentage in than For example, death (SGA 7.2% vs. appropriate (AGA) 2%, p < 0.0001), weeks 42.1% AGA 16.4%, HDP 21.2% 13.5%, = 0.015) NNU 69% 24%, when (perinatal death: 2% 1%, 0.029), weeks: 20.6% 17.4%, 0.020), admission: 34.5% 23.9%, 0.000). There was no association between (p 0.696). In infants, according charts, incidence abnormal umbilical artery Doppler significantly common chart (27.0% 8.1%, 0.001). conclusion, misclassify large at restriction. evidence suggests that could reduce medical postnatally.

Comparing Perinatal Outcome in Twin and Singleton Pregnancies Regarding Doppler Evaluation of Uterine Artery Indices in the Second Trimester

Background: Prediction of adverse perinatal and postnatal outcomes could be possible by using ultrasound Doppler evaluation, however there are some controversies regarding this issue in the literature. The goal of present study was to evaluate predictive value of Doppler indices in single and twin pregnancies during second trimester. Materials and Methods: This prospective study was conducted i...

Is Gestational Hypertension Protective against Perinatal Mortality in Twin Pregnancies?

BACKGROUND Pregnancy-induced or gestational hypertension is a common pregnancy complication. Paradoxically, gestational hypertension has been associated with a protective effect against perinatal mortality in twin pregnancies in analytic models (logistic regression) without accounting for survival time. Whether this effect is real remains uncertain. This study aimed to validate the impact of ge...

Comparison of perinatal outcomes in late preterm birth between singleton and twin pregnancies

OBJECTIVE To determine whether late preterm twin neonates have a more favorable perinatal outcome than singleton late preterm neonates. METHODS We studied 401 late preterm births between 34+0 and 36+6 weeks of gestation, from January 2011 to December 2014 in our institution. We compared the maternal and neonatal characteristics and perinatal outcomes between singleton and twin pregnancies. Pe...

Comparison of the performance of screening test for gestational diabetes in singleton versus twin pregnancies

OBJECTIVE We compared the performance of the 50-g glucose challenge test (GCT) in singleton versus twin pregnancies and investigated the need for adjusting GCT cutoff values for gestational diabetes mellitus (GDM) in twin pregnancies among Korean women. METHODS A retrospective chart review was performed in women who underwent GCT at 24 to 28 weeks' gestation and delivered in our department be...

Diabetes and Perinatal Mortality in Twin Pregnancies

BACKGROUND Diabetes in pregnancy has been associated with a paradoxically reduced risk of neonatal death in twin pregnancies. Risk "shift" may be a concern in that the reduction in neonatal deaths may be due to an increase in fetal deaths (stillbirths). This study aimed to clarify the impact of diabetes on the risk of perinatal death (neonatal death plus stillbirth) in twin pregnancies. METHO...
