Penicillin allergy de-labeling by primary care physicians

Inaccurate penicillin allergy (PCN-A) labels pose a major public health challenge. De-labeling in primary care is crucial but underutilized. We demonstrate an effective model for PCN-A evaluation and de-labeling by physicians (PCPs) adult clinic. PCPs identify refer patients with label to the clinic pharmacist, who risk stratifies using validated, institutional beta lactam stratification decision support algorithm. Risk status conveyed PCPs, counsel accordingly. Patients no are de-labeled based on history. Low individuals scheduled follow up nurse practitioner oral amoxicillin Moderate referred Allergy/Immunology evaluation. Fourteen were evaluated between July 7th August 24th, 2021.
