At its introduction in the management of pediatric asthma, montelukast was regarded as a potentially revolutionary drug due to mechanism action and easy clinical applicability. Nevertheless, use daily practice evidence from trials have shown that, rather than radical change approach asthmatic children, more likely represents second-line medication that is useful when inhaled steroids alone fail providing adequate symptom control. Furthermore, increasingly reported side effects raised concerns regarding safety. In last decade, several studies tried better define strengths drawbacks both preschool wheezing school-age asthma. The present review summarizes literature published on this topic since 2010, highlighting often-controversial results unanswered questions role Moreover, advances understanding mechanisms are reported. main finding emerging analysis application be subset children large groups patients. Future should focus identification biomarkers able predict which patients will benefit achieve tailored prescription.
