Page Kidney: A Secondary Cause of Hypertension

Page kidney is a rare cause of secondary hypertension in adults that occurs due to extrinsic compression the subcapsular collection, such as hematoma or urinoma. Usually these formations are result trauma from biopsy motor vehicle accident. Here we present case 61-year old African American male who presented hospital with worsening shortness breath for 2 days, blood pressure 203/156, and BNP 206. He was admitted treatment begun diastolic congestive heart failure hypertensive emergency. Clinically, he improving but his systolic continued be 150’s despite multiple medications. Due this difficulty controlling negative renal ultrasound, an abdominal CT scan contrast ordered revealed fluid collection indenting lateral margin left measuring approximately 7.1 x 5.4 2.3 cm suggestive per radiology. Upon further history taking it found patient has had received extracorporeal shock wave lithotripsy nephrolithiasis past several months which known risk factor formation. Ultimately determined resolving on its own no intervention needed other than follow-up outpatient. We feel noteworthy not only being presentation kidney, also demonstrates importance considering patients difficult control hypertension.
