PA20 Assessing the value of transition readiness assessment clinics within a dermatology service

Abstract Successful transition of healthcare from paediatric to adult services is a key component developmentally appropriate for young people with long-term conditions. Poor preparation transfer and shifting social roles results in up 25% individuals regularly missing follow-up visits poor treatment adherence once transferred services.
