P22: CONSERVATIVE MANAGEMENT OF NECK OF FEMUR FRACTURES

Abstract Introduction According to NICE CG124, the gold standard of treatment for neck femur (NOF) fractures is operative management on day or after admission. In practice, this not always achievable depending various factors, e.g. patient preference, clinical condition and theatre availability, with a further subset patients undergoing conservative management. Method A retrospective analysis was performed all NOF managed non-operatively at single centre from January 2017 August 2019 by case notes review radiographical images. Demographics, fracture type, pre-fracture mobility status, co-morbidities, mortality Nottingham Hip Fracture Score (NHFS) are reported. Where applicable, our dataset correlated National Database comparison. Result 24 were included in study period, mean age 79.9 years (range 54-99 years) male:female ratio 0.85:1. The NHFS (n=18) 5.7 3-8) least 62.5% (n=15) deemed unfit surgery arrested pre-operatively. Conservatively associated 30-day 41.7% compared average 8.2% national 6.9% fractures. Overall within 1 year 95.5% (n=22). Conclusion Our confirms that high mortality, however it may be only option acceptable who limited unfavourable conditions. Therefore, decision requires careful consideration need an established decision-making pathway. Take-home message therefore
