P18 Value of pneumococcal PCR in respiratory samples for exclusion of pneumococcal pneumonia

Abstract Background Streptococcus pneumoniae is the main aetiological agent in bacterial pneumonia, making it an essential component of respiratory multiplex PCR panels. Despite its relevance, test results are difficult to interpret due frequent non-pathogenic colonization on mucosal surface upper airways. Methods A retrospective study was conducted for lytA-gene as target S. hospitalized patients that were diagnosed with pneumonia and which a panel performed. Patients classified probable, possible or unlikely having pneumococcal pneumonia. Results No difference semi-quantitative Ct-values could be observed between different groups. Sensitivity 71.4% specificity 89.6% found, corresponding negative predictive value positive 97.6% 34.2%, respectively, when considering probable versus possible/unlikely. Conclusions We conclude qPCR good marker inability culture sample possibly sufficient exclude causative agent, we can regard relevant addition syndromic panel.

In our laboratory, culture of sputum was extremely useful in diagnosing the etiology of pneumococcal pneumonia. Of 31 consecutive patients with bacteremic pneumococcal pneumonia, 29 (94%) had Streptococcus pneumoniae cultured from sputum. Recovery of pneumococci in culture was enhanced by anaerobic incubation as well as by a plate bile test and an optochin disk on a primary blood agar plate.
