P.037 Neurofascin-155 IgG in acute-onset inflammatory polyneuropathy: possible predictor of relapse and recovery

Background: IgG4 autoantibodies to neurofascin-155 (NF-155) have been described in a subset of patients with chronic inflammatory demyelinating polyneuropathy (CIDP). While reports suggest an acute onset is more likely than antibody negative CIDP, little literature exists around the subsequent course NF-155 positive cases that originally presented (AIDP) phenotype. Methods: Two male patients, ages 51 and 59, similar, <2 week histories lower extremity weakness. Patients were diagnosed AIDP treated IVIG. Following initial improvement, both relapsed. One patient was IVIG steroids improvement; however, he unable be weaned from without experiencing recurrence symptoms. The other not retreated. Testing for IgG sent. Results: first ultimately required Rituximab stable improved spontaneously. Both later had tests antibodies. Conclusions: our AIDP-like presentations, followed by relapsing excellent eventual recovery. This result, along limited available cases, presentation, could marker disease recurrence, but do necessarily predict poor outcome.
