P-129 TRAUMATIC HERNIA, URGENT REPAIR?

Abstract Aim We present our clinical experience in the urgent repair of traumatic hernias through two cases. Material and Methods Case 1: A 45-year-old man suffering thoraco-abdominal trauma due to running over. Thoracoabdominal CT scan surgical examination shows a meso tear terminal ileum section right lateral musculature. 2: 19-year-old abdominal traffic accident. abdomen is performed which reports large hernia left anterior wall including intestinal loops, with pneumo retroneumoperitoneum. Results close mesenteric gap musculature by associating polypropylene mesh preperitoneal space. The patient discharged after 13 days. During two-year follow-up, no recurrence was detected. reconstruction posterior separation components, addition ileocecal sigma resection anastomosis. As complication presents seroma chronic sinus, without during follow-up for 5 years. Conclusions tendency their second time attending first injuries that compromise survival patient, but larger defect can lead irreversible sequelae since enlargement hernia, atrophy muscle retraction, resulting more complex repair. If requires intervention visceral injury, some authors defend simultaneous stable patients. Our group thinks patients individualized management its will depend on situation.
