P-097 FEMORAL AND OBTURATOR HERNIA INCARCERATED: A CASE REPORT

Abstract Obturator hernia is a very rare type of abdominal wall hernia, accounting for less than 1% all hernias. They can be confused with femoral hernias as well they arise simultaneously, just like in this clinical case. A 82 year old female, multiple comorbidities including right inguinal hernioplasty, was brought to our emergency department complaining pain, distension and vomiting the last 24 hours. On physical examination, there where abnormally active, high-pitched bowel sounds, pain on left lower quadrant but no tenderness. Palpation groin area showed an incarcerated apparently containing only omentum. As patient maintained we proceed CT scan, which demonstrated small dilation air-fluid levels consistent obstruction originating from obturator hernia. Open tension-free mesh repair done need ressection since it contained omentum, without ischemic signs. transperitoneal nonmesh performed after reduction. There irreversible ischemia. Evolution during hospitalization uneventful. Regarding hernias, optimal approach so probably depends condition surgeon experience. preferred when there's signs obstruction,
