چکیده
Abstract Study question What is the frequency of clinically significant results from expanded carrier screening (ECS) in a sperm donor applicant population and how should these be managed? Summary answer ECS revealed information for approximately 1 51 applicants, which warrant specific management considerations context third-party reproduction. known already The American Society Reproductive Medicine (ASRM) Practice Committee published guidance January 2021 that outlines recommended approach gamete donors. However, there no additional direction related to including suitability, counseling, informed consent. Prior studies have illustrated high donors who are identified as carriers one or more recessive disorders on an panel; thus, excluding all prospective conditions not feasible appropriate given availability reciprocal reproductive partner. design, size, duration A retrospective review genetic records July 2017 December was performed. Relevant data extracted categorized by result. Genetic counselors evaluated health risks potential (PDs) associated with being pathogenic mutation each gene using data, reference laboratory interpretations, professional guidelines. Participants/materials, setting, methods performed applicants part routine qualification process. Testing at outside after participants provided written consent testing. genes included panel were analyzed multiple methodologies. Specific methodologies varied based upon offerings time entered program. Main role chance total 966 PDs had during specified timeframe. Of 19 (1.97%) having potentially their results. those PDs, eleven found either heterozygous hemizygous may convey carriers. these, nine positive variant typically AR condition two carried variants X-linked conditions. Eight compound homozygous autosomal condition. Limitations, reasons caution Donor recruitment limited select major metropolitan areas U.S.; represent ethnic groups socioeconomic backgrounds. Additionally, this study described single does illustrate variation panels across testing laboratories. Wider implications findings While examined population, also applicable general necessity post-test counseling. In some cases, provide insight future opportunities. Trial registration number Not

Factors for successful implementation of population-based expanded carrier screening: learning from existing initiatives
Background Carrier screening for autosomal recessive disorders aims to facilitate reproductive decision-making by identifying couples with a 1-in-4 risk in every pregnancy of having an affected child. Except for a few countries or regions, carrier screening is not widely offered and is mostly ancestry-based. Technological advances enable carrier screening for multiple diseases simultaneously al...

OBJECTIVE To discuss the implications of expanded genetic carrier screening for preconception purposes based on our practice. METHODS One hundred and forty-three potential gamete donors aged 20-32 years old (µ=24, 127 females and 16 males), signed informed consent forms and were selected according to the REDLARA guidelines. Blood or saliva samples were examined by one of these genetic carrier...
متن کاملCarrier Screening is a Deficient Strategy for Determining Sperm Donor Eligibility and Reducing Risk of Disease in Recipient Children
AIMS DNA-based carrier screening is a standard component of donor eligibility protocols practiced by U.S. sperm banks. Applicants who test positive for carrying a recessive disease mutation are typically disqualified. The aim of our study was to examine the utility of a range of screening panels adopted by the industry and the effectiveness of the screening paradigm in reducing a future child's...
متن کاملValidation for clinical use of, and initial clinical experience with, a novel approach to population-based carrier screening using high-throughput, next-generation DNA sequencing.
Traditional carrier screening assays are designed to look for only the most common mutations within a gene owing to cost considerations. Although this can yield high detection rates in specific populations for specific genes (such as cystic fibrosis in Caucasians), they are suboptimal for other ethnicities or for patients of mixed or unknown ethnic background. Next-generation DNA sequencing pro...
متن کاملPublic Health and Medical Perspectives on Population-Based Carrier Screening
Population-based carrier screening for single gene disorders has been occurring since the 1960s, with each decade bringing new opportunities and challenges. In February 2008, approximately 125 scientists, clinicians, public health officials, and patient representatives gathered to consider population-based carrier screening in light of lessons learned and emerging technologies. The following do...
